Advances in Hormone-Related Cardiovascular Disorders: Mechanisms, Diagnostics, and Therapeutic Innovations

Hormones are key regulators of cardiovascular structure and function across the lifespan. Emerging evidence highlights a bidirectional endocrine–cardiac axis: the heart acts not only as a hemodynamic pump but also as an endocrine organ secreting natriuretic peptides and other cardiokines, while classical endocrine pathways—renin–angiotensin–aldosterone system (RAAS), mineralocorticoids, thyroid hormones, sex steroids, insulin/IGF signaling, and melatonin—shape vascular tone, myocardial remodeling, and metabolic homeostasis. Disruptions in these axes underlie diverse conditions, from endocrine hypertension and atherosclerosis to heart failure with preserved ejection fraction (HFpEF), peripheral arterial disease (PAD), arrhythmias, microvascular dysfunction, and cardiometabolic syndromes. This Research Topic invites basic, translational, and clinical studies that elucidate hormone-related mechanisms and deliver diagnostic and therapeutic advances. We welcome work on: (i) developmental and life-course endocrinology influencing cardiovascular risk; (ii) endocrine drivers of hypertension, dyslipidemia, thromboinflammation, and vascular aging; (iii) sex-specific cardiometabolic risk including the impact of oral hormonal contraceptives and menopausal transitions; (iv) pleiotropic endocrine effects of established drugs (e.g., statins, SGLT2 inhibitors, GLP‑1 receptor agonists, MRAs) and novel agents; (v) autophagy, mitochondrial quality control, and oxidative stress as hormonal effector pathways, including the role of melatonin; and (vi) biomarker discovery and multimodal diagnostics, integrating natriuretic peptides, steroid profiles, omics, and imaging. We particularly encourage submissions that bridge mechanism to practice: targetable nodes within RAAS and mineralocorticoid signaling; endocrine phenotyping to personalize antihypertensive therapy; strategies to mitigate vascular and thrombotic risk with contraceptives; repurposing of endocrine modulators in PAD and heart failure; and circadian/endocrine alignment therapies. Article types may include Original Research, Brief Research Report, Methods, Review/Mini‑Review, Systematic Review, Clinical Trial, and Perspective. By converging endocrine biology with cardiovascular pharmacology, this Topic aims to accelerate precision prevention and treatment of hormone-related cardiovascular disorders. We welcome submissions that address, but are not limited to, the following themes:• Endocrine Hypertension and Vascular RemodelingRAAS/mineralocorticoid pathways, salt sensitivity, fibrosis, and microvascular rarefaction; endocrine biomarkers guiding therapy.• Sex Hormones, Contraceptives, and Thrombo-Cardiometabolic RiskEstrogen/progestin formulations, coagulation, lipids, BP, arrhythmia risk; risk stratification and safer regimens.• Metabolic–Endocrine Interfaces in Atherosclerosis and PADInsulin resistance, adipokines, thyroid dysfunction; anti-inflammatory and lipid-modulating strategies, including statin pleiotropy.• Cardiomyocyte and Vascular Autophagy as Hormonal Effector PathwaysMelatonin, thyroid, and glucocorticoid signaling in autophagy/mitophagy, oxidative stress, and cardioprotection.• The Heart as an Endocrine Organ: Biomarkers and Therapeutic TargetingNatriuretic peptides and cardiokines for diagnosis, prognosis, and drug development; omics-driven endocrine phenotyping.
