Advancing Cardiac Implantable Electronic Devices: Leadless and Beyond

Cardiac implantable electronic devices (CIEDs) have evolved rapidly, transforming cardiac rhythm management. Beyond traditional transvenous pacemakers and defibrillators, emerging systems now include leadless pacemakers, subcutaneous and extravascular ICDs, conduction system pacing, and wirelessly coordinated or modular platforms. These technologies aim to reduce procedural risks, enhance physiologic pacing, and enable remote or intelligent monitoring, marking a shift toward a connected and minimally invasive device era.Despite progress, unresolved challenges remain. Device-related complications, retrieval and upgrade logistics, data interoperability, cybersecurity, and disparities in global access continue to limit widespread adoption. Evaluating performance, safety, and integration of next-generation systems—and ensuring their equitable and sustainable deployment—will be essential. Bridging the gap between engineering innovation and clinical application offers new opportunities to improve patient outcomes and system efficiency. This Research Topic aims to explore emerging technologies and care pathways shaping the future of CIED therapy. Contributions are invited that assess novel designs and pacing strategies, remote and AI-enabled management, complication prevention, and institutional or policy frameworks supporting safe and equitable implementation. To advance understanding in this field, we welcome work addressing, but not limited to, the following themes:o Long-term performance and retrieval of leadless pacemakerso Comparative outcomes across leadless, subcutaneous, and extravascular systemso Conduction system and physiologic pacing approacheso Wirelessly coordinated and hybrid device platformso Remote monitoring, AI integration, and cybersecurityo CIED infection prevention and complication managemento Transvenous lead extraction and procedural refinemento Institutional protocols and multidisciplinary collaborationo Health economics and global accessibility of next-generation CIEDs Topic editors: Prof. Sergio Dubner, Alberto Alfie Source: https://www.frontiersin.org/research-topics/82347/advancing-cardiac-implantable-electronic-devices-leadless-and-beyond
