Advancing Cellular Immunotherapies in Lymphoid Malignancies: Challenges, Biomarkers, and Future Perspectives
Lymphoid malignancies represent a complex and heterogeneous field within hematologic oncology, encompassing a wide array of disorders such as leukaemia’s and lymphomas. These subtypes differ significantly in their clinical presentation, biological behaviour, and outcomes, necessitating highly individualized diagnostic and management approaches. Contemporary diagnostics have greatly benefitted from advances in immunophenotyping, molecular characterization, and genetic profiling, providing nuanced insights into disease mechanisms and opening avenues for targeted interventions. Nevertheless, despite these innovations, the field faces persistent challenges including disease relapse, treatment resistance, and uncertainty regarding optimal sequencing of novel therapies. In recent years, cellular therapy has emerged as a transformative force in treating lymphoid malignancies. Techniques such as chimeric antigen receptor (CAR) T-cell therapy, bispecific T-cell engagers, and antibody–drug conjugates have delivered remarkable clinical responses, particularly in patients with relapsed or refractory disease. Despite these achievements, critical gaps remain in our understanding of mechanisms underpinning resistance, durability of responses, and the identification of predictive and prognostic biomarkers for patient selection. Additional complexities arise in translating these breakthroughs from clinical trials to the real world, where variations in practice, toxicity management, and patient populations may influence outcomes. As the therapeutic landscape becomes more intricate, head-to-head comparisons and long-term follow-up studies are urgently required to inform rational, evidence-based integration of cellular therapies into routine clinical care. This Research Topic aims to synthesize current knowledge and stimulate comprehensive investigation into the evolving role of cellular therapy in lymphoid malignancies. By fostering multidisciplinary exchange and highlighting translational advances, it seeks to address core questions regarding efficacy, resistance, optimal sequencing, and integration of cellular approaches with established treatments. Special emphasis is placed on bridging the gap between experimental innovation and clinical practice, ultimately aiming to improve patient outcomes and inform future therapeutic directions. To gather further insights in the biological, translational, and clinical dimensions of lymphoid malignancy management, we welcome articles addressing, but not limited to, the following themes:• Advances in diagnostic and prognostic biomarker development• Mechanisms of resistance to cellular therapies and novel agents• Real-world clinical outcomes and toxicity management in cellular immunotherapy• Comparative effectiveness and sequencing strategies for cellular and targeted approaches• Integration of cellular therapies into current treatment algorithms• Long-term follow-up and survivorship considerations in lymphoid malignancies• Healthcare delivery challenges, patient perspectives, and implementation of novel therapies Topic editors: Mamtha Balla, Nishanth Thalambedu, Sumit Gupta, Barath Prashanth Sivasubramanian Source: https://www.frontiersin.org/research-topics/74407/advancing-cellular-immunotherapies-in-lymphoid-malignancies-challenges-biomarkers-and-future-perspectives
