Altered cardiovascular control and physiological adaptations to exercise in cardiometabolic disorders
Cardiovascular physiology is a central component to understanding how the human body responds and adapts to exercise. In individuals with cardiometabolic disorders—such as obesity, insulin resistance, metabolic syndrome, and type 2 diabetes— the ability to coordinate cardiac output, vascular conductance, and autonomic control during exercise is often impaired. Recent studies have unveiled links between the magnitude of sympathetic activation, endothelial nitric oxide availability, and limitations to skeletal muscle oxygen delivery. However, the integrated physiological mechanisms governing these alterations remain incompletely understood, as do their collective influence on exercise tolerance. Thus, despite growing research in this area , there remains a need for a better understanding of how central and peripheral cardiovascular regulation interacts across the cardiometabolic continuum and how these mechanisms contribute to exercise limitation and recovery. This Research Topic aims to advance mechanistic knowledge of cardiovascular responses to exercise under conditions of cardiometabolic dysfunction. Specifically, it seeks to clarify how dysregulation in autonomic, vascular, and microcirculatory function affects acute and chronic exercise responses. By integrating human and preclinical investigations, mechanistic interventions, and studies utilizing experimental models of exercise stress, this Research Topic strives to identify physiological determinants of impaired exercise tolerance and training adaptation. Ultimately, it intends to inform evidence-based approaches for optimizing exercise protocols aimed at improving vascular and metabolic health. To gather further insights into the physiological mechanisms regulating cardiovascular responses to exercise in cardiometabolic dysfunction, we welcome articles addressing, but not limited to, the following themes:o Autonomic cardiovascular regulation and sympathetic-parasympathetic balance during exerciseo Endothelial function, nitric oxide bioavailability, and vascular conductance mechanismso Microvascular oxygen delivery and skeletal muscle perfusion dynamicso Hemodynamic adjustments and baroreflex function during acute exercise and recoveryo Physiological determinants of exercise intolerance and adaptive remodelingo Comparative responses between aerobic, resistance, and intermittent exercise modalitieso Integrative models linking metabolic status, circulatory regulation, and exercise capacityWe invite original research, mechanistic studies, and translational investigations that illuminate how physiological processes govern exercise responses in cardiometabolic dysfunction, consistent with the Exercise Physiology section’s focus on molecular, cellular, and systemic integration.
