Atrioventricular Valve Disease and Cardiac Arrhythmias: Mechanisms and Integrated Treatment
Atrioventricular valve disease and cardiac arrhythmias are closely interconnected cardiovascular conditions with important implications for diagnosis, prognosis, and treatment. Mitral and tricuspid regurgitation can promote atrial and ventricular dilation, pressure and volume overload, neurohormonal activation, and myocardial fibrosis, thereby creating an arrhythmogenic substrate. Conversely, atrial and ventricular arrhythmias may worsen valve dysfunction through impaired contractility, altered loading conditions, and tachycardia-mediated remodeling. Current evidence supports a bidirectional relationship, but important questions remain regarding the mechanisms that determine which patients develop clinically significant arrhythmias, how rhythm disturbances influence disease progression, and whether early rhythm or valve intervention can alter long-term outcomes. Advances in echocardiography, cardiac magnetic resonance, electrophysiology, catheter-based procedures, and surgical techniques are improving the characterization and treatment of these conditions. However, the optimal timing and sequencing of rhythm and structural interventions remain uncertain, particularly in patients with complex, multivalvular, or secondary disease. Further investigation is needed to integrate mechanistic insights, imaging findings, and clinical outcomes into personalized management strategies. This Research Topic aims to advance understanding of the biological and clinical interactions between atrioventricular valve disease and cardiac arrhythmias. It will examine how valve dysfunction contributes to electrical and structural remodeling, how arrhythmias affect valve function and patient outcomes, and how contemporary rhythm-control and structural interventions can be coordinated. Contributions may also address predictors of treatment response, the role of multimodality imaging and electrophysiological assessment, and strategies for improving individualized decision-making across the disease course. To gather further insights into the interplay between atrioventricular valve disease and cardiac arrhythmias, we welcome articles addressing, but not limited to, the following themes:• Molecular, cellular, and hemodynamic mechanisms linking atrioventricular valve dysfunction with arrhythmogenesis.• Atrial and ventricular remodeling, fibrosis, mechanical dispersion, and electrophysiological substrate characterization.• Echocardiographic, cardiac magnetic resonance, computed tomography, and electroanatomical imaging markers.• Screening, risk stratification, and prediction of arrhythmia development in patients with valve disease.• Pharmacological, catheter-based, and surgical rhythm-control strategies.• Outcomes of transcatheter and surgical mitral or tricuspid valve interventions on arrhythmia burden.• Combined or staged electrophysiological and structural interventions.• Management of atrial fibrillation, atrial flutter, ventricular arrhythmias, and conduction disorders in the setting of valve disease.• Timing and sequencing of valve repair, valve replacement, ablation, and device therapy.• Multidisciplinary decision-making, patient selection, and individualized care pathways.• Prognostic implications of integrated valve and rhythm management.*Topic editor Gerrit Kaleschke is on the advisory board of Medtronic.
