Beyond One-Size-Fits-All: Precision Medicine in Chronic Cardiovascular Care

Most patients with heart failure, cardiometabolic disease, or multimorbidity are still managed through standardised pathways built for the average patient, not the one in front of you. The tools to do better already exist: phenotyping, biomarkers, imaging, digital health platforms, wearables, and remote monitoring. The bottleneck is no longer innovation. It is implementation. This shift defines where chronic cardiovascular care stands today. The question is not whether precision medicine is achievable. It is how to embed it into ambulatory care, long-term management, and routine clinical settings in ways that are practical, scalable, and sustainable for real-world practice. This Research Topic brings together clinical, translational, and health systems research that bridges that gap. We welcome work that shows, concretely, how more individualised approaches can improve diagnosis, treatment selection, monitoring, and follow-up for patients with chronic cardiovascular conditions, particularly those living with overlapping diseases and complex trajectories that standardised pathways fail to capture.Contributions that centre clinical applicability, patient stratification, care pathway redesign, and real-world impact are especially encouraged.Topics of interest include, but are not limited to:• Phenotype-guided management in heart failure, cardiometabolic disease, and chronic cardiovascular syndromes;• Identification of clinically actionable traits that support individualized treatment selection;• Integration of biomarkers, imaging, and physiological data into clinical decision making;• Personalized follow-up strategies and risk guided long term management;• Remote monitoring, wearable technologies, and digital health tools in chronic cardiovascular care;• Multimorbidity and its implications for tailored cardiovascular management;• Clinical implementation of precision cardiology in outpatient and routine care settings;• Evaluation of precision-based care pathways, service models, and healthcare delivery strategies;• Decision support tools and data driven approaches when clearly embedded in clinical care and implementation contexts;• Real world studies, pragmatic evaluations, and health systems research supporting individualized cardiovascular care;We welcome submissions that advance precision medicine in chronic cardiovascular care.
