Bladder Cancer Across the Spectrum: Contemporary Approaches to Non-Muscle-Invasive and Muscle-Invasive Disease
Bladder cancer is the most common malignancy of the urinary tract and presents across a wide clinical and biological spectrum, from low-grade non-muscle-invasive tumors to locally advanced and metastatic disease. Despite meaningful therapeutic advances, management decisions at each stage remain highly contested. In non-muscle-invasive bladder cancer (NMIBC), the emergence of FDA-approved intravesical novel agents for BCG-unresponsive disease has reshaped the treatment landscape, while evolving risk stratification tools and prospective trial data are actively challenging traditional escalation and de-escalation paradigms. In muscle-invasive bladder cancer (MIBC), the optimal integration of perioperative systemic therapy — including immune checkpoint inhibitors — with radical cystectomy or bladder-preserving trimodal therapy continues to be refined, and surgical quality metrics, urinary diversion techniques, and functional outcomes remain areas of intense investigation. The intersection of these two disease states — particularly the management of patients with recurrent high-risk NMIBC and the decision to pursue radical cystectomy versus continued bladder-sparing strategies — represents one of the most clinically consequential and data-rich areas of modern urologic oncology. This Research Topic aims to assemble a comprehensive and clinically actionable collection of original research, systematic reviews, clinical trial reports, and perspective pieces that advance our understanding of bladder cancer management across its full spectrum. We welcome submissions addressing, but not limited to, the following themes:- Risk stratification and biomarker-driven patient selection in high-risk NMIBC- BCG optimization, BCG-unresponsive disease, and novel intravesical and systemic salvage strategies- Patient selection and timing of radical cystectomy in high-risk and recurrent NMIBC- Patient-reported outcomes and quality of life following radical cystectomy versus bladder-preserving therapy- Perioperative systemic therapy in MIBC: neoadjuvant and adjuvant chemotherapy and immunotherapy- Trimodal bladder-preserving therapy: patient selection, oncologic outcomes, and functional results- Surgical technique, urinary diversion, and reconstruction: outcomes and quality of life- Healthcare disparities and real-world patterns of care in NMIBC and MIBC- Multidisciplinary decision-making frameworks at the NMIBC-MIBC interface Topic editors: Ali Mouzannar, Andrew Katims, Chad Ritch Source: https://www.frontiersin.org/research-topics/80636/bladder-cancer-across-the-spectrum-contemporary-approaches-to-non-muscle-invasive-and-muscle-invasive-disease
