Cardiac Sarcoidosis: Bridging Knowledge Gaps in Risk Stratification, Treatment, and Outcomes

Cardiac sarcoidosis is an emerging field within cardiovascular medicine, marked by its variable clinical presentations and potentially severe outcomes, including high-grade atrioventricular block, ventricular arrhythmias, heart failure, and sudden cardiac death, especially among young adults. Historically considered both rare and difficult to diagnose, the increased accessibility and use of advanced imaging techniques—such as cardiac MRI and FDG-PET scans—have transformed the landscape by enabling earlier and more accurate detection. Despite these advances, significant uncertainties persist regarding optimal management strategies, patient selection for device implantation, and identification of individuals at highest risk for poor outcomes. Recent studies have helped delineate clinical and imaging features associated with prognosis, but clear guidelines for therapy and risk assessment are lacking. While some management approaches, such as immunosuppression and device therapy, are employed, their timing, selection, and long-term efficacy remain under debate, and consensus on who should undergo screening is still evolving. This Research Topic aims to address critical knowledge gaps in the diagnosis, risk stratification, monitoring, and management of cardiac sarcoidosis. By encouraging novel research, case studies, reviews, and expert perspectives, this topic seeks to clarify best practices for patient care, illuminate the utility of emerging diagnostic tools, and define high-risk features across demographic, clinical, and imaging domains. The objective is to foster a comprehensive understanding of cardiac sarcoidosis that translates into improved, evidence-based clinical decision-making and ultimately, better outcomes for affected individuals.Given the complexity and evolving nature of this condition, the scope is limited to original research, clinical studies, and review articles that explore diagnostic approaches, risk markers, therapeutic interventions, and prognostication in cardiac sarcoidosis. To gather further insights in this dynamic research area, we welcome articles addressing, but not limited to, the following themes:• Early diagnosis and advances in imaging for cardiac sarcoidosis;• Risk stratification: demographic, clinical, and imaging markers of adverse outcomes;• Evidence and controversies in immunosuppressive therapies and other pharmacological treatments;• Indications for cardiac device implantation: pacemakers and defibrillators• Screening strategies in high-risk populations;• Prognostic models and long-term follow-up data;• Real-world challenges and unmet needs in patient care.
