Cardio-Renal Interactions in Diabetes: Mechanistic Insights and Clinical Implications

The bidirectional relationship between cardiovascular and renal dysfunction represents one of the most clinically significant challenges in diabetes management. Patients with diabetes frequently develop both cardiac and kidney complications, with each organ influencing the progression and prognosis of the other. Despite substantial advances in pharmacotherapy, the precise mechanisms underlying this cardio-renal interplay remain incompletely understood, particularly in the context of oxidative stress, endothelial dysfunction, metabolic inflammation, and insulin resistance.Recent clinical and experimental findings have begun to reveal how molecular pathways such as protein kinase C (PKC) activation, impaired nitric oxide signalling, and inflammatory cytokine release contribute to this complex axis. Moreover, novel glucose-lowering therapies, including SGLT2 inhibitors and GLP-1 receptor agonists, have demonstrated beneficial effects across both cardiovascular and renal outcomes, highlighting the therapeutic importance of understanding shared mechanisms. This Research Topic aims to explore the integrative mechanisms and clinical implications of cardio-renal interactions in diabetes. By bridging molecular endocrinology and clinical practice, it encourages submissions that enhance understanding of the pathophysiology, diagnosis, and management of diabetes-related cardiac and renal complications.Topics of interest include:• Mechanistic pathways linking cardiac and renal dysfunction in diabetes • Inter-organ communication and metabolic crosstalk underlying the cardio-renal axis• Clinical studies evaluating therapeutic interventions (e.g, SGLT2 inhibitors, GLP-1 receptor agonists) and safety considerations across kidney risk categories• Biomarkers and imaging-based methods for early detection and monitoring of cardio-renal complications• Impact of insulin resistance, and metabolic inflammation on the progression of cardio-renal disease• Translational studies integrating mechanistic findings with patient outcomes and clinical decision-making Topic editors: Yuzuru Ohshiro, Kunimasa Yagi Source: https://www.frontiersin.org/research-topics/77332/cardio-renal-interactions-in-diabetes-mechanistic-insights-and-clinical-implications
