Cardiovascular Risk Stratification Across the Coronary Artery Disease Spectrum: Acute, Chronic, and Perioperative Perspectives

Coronary artery disease does not follow a tidy clinical narrative. The same patient may present with chronic coronary syndromes, experience an acute event, stabilise on guideline-directed medical therapy, and later face major noncardiac surgery, carrying elevated and often unquantified risk at every transition. Despite decades of progress in pharmacotherapy, revascularisation, and imaging, individual outcome prediction remains imprecise, particularly in the growing population of complex, comorbid patients who span multiple clinical contexts at once. This Research Topic invites work that treats CAD as what it is: a continuum. We welcome contributions that move beyond single-setting analyses to connect risk stratification and outcome prediction across the full disease spectrum, spanning chronic coronary syndromes and acute coronary syndromes, revascularisation decisions in complex and structurally diseased hearts, and perioperative care for patients undergoing noncardiac surgery. The goal is an evidence base that supports genuinely personalised clinical decisions rather than solely population-level recommendations. We welcome original research, systematic reviews, meta-analyses, clinical trials, and perspectives addressing:• Risk stratification in chronic coronary syndromes and stable CAD;• Coronary artery disease in patients with structural and valvular heart disease: concomitant risk assessment and management decisions;• Revascularisation strategies and long-term outcomes in complex CAD: PCI, CABG, and hybrid approaches;• Preoperative cardiovascular assessment in patients with known or suspected CAD;• Perioperative myocardial injury and infarction after noncardiac surgery (MINS/PMI);• Biomarkers (high-sensitivity troponin, NT-proBNP, inflammatory markers) for risk prediction across acute, chronic, and perioperative settings;• Multimodality imaging (coronary CT angiography, echocardiography, CMR, nuclear) for prognostic assessment;• Integration of frailty, comorbidity burden, and patient-reported outcomes into prediction models;• Development, validation, and implementation of clinical risk scores and decision-support tools;• Long-term outcomes and secondary prevention in mixed CAD populations.
