Cerebral amyloid angiopathy, new insights into an evolving dynamic small vessel disease

Cerebral amyloid angiopathy (CAA) is a critical vascular disorder in aging, recognized as a major cause of lobar intracerebral hemorrhage and a significant driver of cognitive and motor dysfunction in the elderly. Despite growing awareness, the diagnosis and management of CAA remain complex due to its dynamic interplay with Alzheimer’s disease and other vascular brain pathologies. Contemporary research highlights the heterogeneous clinical and imaging presentations of CAA, as well as emerging complexities related to amyloid-targeting therapies. Advances in neuroimaging, fluid biomarkers, and computational analysis have improved our ability to delineate CAA subtypes, track progression, and detect treatment-related changes like amyloid-related imaging abnormalities (ARIA). However, fundamental questions persist regarding the biological underpinnings of disease heterogeneity, reliable risk assessment, personalized care strategies, and optimal clinical trial design. Bridging these gaps is crucial for translating scientific progress into meaningful outcomes for patients at risk or living with CAA.This Research Topic aims to catalyze translational and clinically actionable research across the spectrum of CAA, from mechanistic insights and biomarker development to prognosis, stratification, and individualized management strategies. The objective is to foster an integrated understanding of CAA’s diverse phenotypes, the evolution of disease activity, predictors of clinical outcomes, and responses to emerging therapies. Specific aims include clarifying the utility of multimodal biomarkers, refining risk prediction tools, and exploring patient-centered interventions that may mitigate hemorrhagic and cognitive complications. The initiative encourages innovative approaches leveraging longitudinal cohorts, real-world data, and advanced analytics to unravel the complex trajectories of CAA.To gather further insights in the pathophysiology, diagnosis, and management of CAA, this Research Topic will focus on human studies that engage translational, diagnostic, and therapeutic aspects, while excluding preclinical or purely basic science investigations. We welcome articles addressing, but not limited to, the following themes:- Multimodal neuroimaging advances for CAA diagnosis and monitoring- Fluid biomarkers for differentiating CAA and tracking disease activity- Computational and machine learning methods in risk stratification and prognosis- Interaction of CAA with Alzheimer’s and other neurovascular conditions- Clinical implications and management of amyloid-related imaging abnormalities (ARIA)- Patient-centered approaches to cognitive and functional impairment in CAA- Design and implementation of CAA-targeted clinical trials in real-world contexts Topic editors: John Ly, Hsin-Hsi Cynthia Tsai, Ulf Jensen-Kondering, Nicolas Raposo Source: https://www.frontiersin.org/research-topics/77627/cerebral-amyloid-angiopathy-new-insights-into-an-evolving-dynamic-small-vessel-disease
