Complications of Carotid Interventions: Presentation and Contemporary Management

Carotid endarterectomy and carotid artery stenting remain cornerstone interventions for stroke prevention in patients with significant carotid artery disease. While both techniques have well-established efficacy, their benefit depends critically on minimizing perioperative and postoperative complications. Hemodynamic instability, cerebral hyperperfusion syndrome, cranial nerve injury, and restenosis continue to challenge even experienced surgical and interventional teams, and their optimal management remains an area of active clinical debate.Postoperative blood pressure control after carotid revascularization is particularly demanding: both hypotension and hypertension carry distinct risks, from watershed cerebral ischemia to hyperperfusion syndrome and intracranial hemorrhage. Similarly, restenosis after carotid intervention presents a recurring management dilemma, with decisions ranging from surveillance to redo endarterectomy, patch angioplasty, or repeat stenting, each carrying its own risk-benefit profile. This Research Topic aims to bring together contemporary evidence and clinical experience on the presentation, prevention, and management of complications following carotid interventions. We welcome contributions addressing, but not limited to, the following aspects:• Postoperative blood pressure management strategies after carotid endarterectomy and stenting• Pathophysiology, risk stratification, and treatment of cerebral hyperperfusion syndrome• Diagnosis and management of carotid restenosis, including redo endarterectomy and patching techniques• Cranial nerve injury and other local complications following carotid surgery• Comparative outcomes between carotid endarterectomy and carotid artery stenting with respect to complication rates• Perioperative physiological monitoring and protocols to reduce complication risk• Long-term follow-up data and secondary prevention after carotid intervention complicationsWe welcome Original Research, Review, Mini Review, Case Report, and Systematic Review articles that advance the understanding of perioperative physiology and complication prevention in carotid surgery and intervention, with the goal of informing safer, more effective contemporary management strategies.
