Controversies in Pediatric Hypoglycemia
Hypoglycemia remains one of the most challenging topics in pediatric endocrinology, primarily due to the lack of a universally accepted consensus across several critical points.Current guidelines focus primarily on newborns and children with Type 1 Diabetes (T1D). In other pediatric populations there is significant variability in clinical practice. This lack of consensus also results in considerable global variation in management strategies. Identifying current controversies in pediatric hypoglycemia is essential to delineate future research priorities, ultimately harmonizing global clinical practices and addressing the diagnostic gaps that persist in pediatric care. The aim of this Research Topic is to highlight the key controversies surrounding pediatric hypoglycemia, featuring selected themes that investigate the current state-of-the-art. By critically examining these areas, this collection intends to highlight existing clinical challenges while identifying areas of shared consensus. Furthermore, this Research Topic seeks to bridge the gap between traditional management and modern approaches, such as continuous glucose monitoring (CGM) and molecular genetics. It also invites discussion on the contested definition of hypoglycemia and whether diagnostic thresholds should be personalized to individual patient and family characteristics. Beyond clinical management, this Topic welcomes contributions on the psychosocial impact of hypoglycemia and the burden of monitoring fatigue experienced by patients and their families. This Research Topic represents the initial phase of a broader initiative aimed at defining future research priorities. Its ultimate goal is to harmonize global clinical practices in pediatric hypoglycemia. Submissions may address and discuss controversies in pediatric hypoglycemia related to the following specific themes:- Screening- Definition- Diagnostic work-up- Dynamic tests- Etiology- Therapeutic approaches- Congenital conditions or syndromes associated with hypoglycemia- Neurological outcomes- Neuroimaging- Continuous glucose monitoring- Molecular geneticsWe welcome the following article types: Mini-Review, Review, Systematic Review, Perspective, General Commentary, and Opinion.
