Critical Cases in Clinical Nephrology - Volume II
Caring for critically ill patients with severe renal complications represents one of the most demanding frontiers in intensive care medicine. Following the success of Volume I, Volume II continues to explore complex clinical presentations, diagnostic dilemmas, and innovative therapeutic interventions across intensive care units and nephrology wards. In contemporary critical care, acute kidney injury (AKI) rarely occurs in isolation; it frequently intersects with multiorgan failure, cardiorenal/hepatorenal syndromes, systemic sepsis, and novel treatment-related toxicities (including immune checkpoint inhibitors and antibody-drug conjugates). Furthermore, the long-term sequelae of critical AKI—specifically the AKI-to-CKD transition—demand systematic tracking and innovative post-ICU management. Management within modern intensive care requires rapid multidisciplinary decision-making, sophisticated hemodynamic monitoring, and the integration of advanced extracorporeal organ support. This collection highlights real-world clinical problem-solving, unexpected disease trajectories, and educational insights to elevate standards of care and support UN Sustainable Development Goal 3 (Good Health and Well-being).he primary goal of Volume II of 'Critical Cases in Clinical Nephrology' is to bridge the gap between idealized clinical trial protocols and the multifaceted, high-stakes realities of bedside patient care. Critical nephrology cases frequently present with atypical clinical features, diagnostic uncertainties, and therapeutic controversies that standard practice guidelines cannot fully address. By assembling detailed, educational, and rigorously documented case reports and case series, this collection provides actionable insights into clinical decision-making, troubleshooting of renal replacement therapies, and management of refractory complications. Volume II places special emphasis on the integration of kidney replacement therapy (KRT) with advanced multiorgan support systems (such as ECMO, ECCO2R, and hemoperfusion), emerging cellular and regenerative therapies for renal repair, point-of-care ultrasound (POCUS), and individualized pharmacotherapy in hypercatabolic critically ill patients. Authors are encouraged to provide comprehensive diagnostic rationales, multidisciplinary care coordination pathways, and detailed literature contextualisation to advance clinical knowledge-sharing.This Research Topic welcomes Case Reports, Case Series, Mini-Reviews, and Clinical Study Protocols focusing on acute, complex, or unusual renal conditions in critically ill and hospitalized patients.Key themes of interest include:- Atypical, refractory, or rapidly progressive acute kidney injury and glomerulopathies.- Practical troubleshooting and novel protocols in continuous kidney replacement therapy (CKRT), hybrid dialysis modalities, and peritoneal dialysis in critical care.- Combined extracorporeal life support: tandem CRRT with ECMO, therapeutic plasma exchange (TPE), and selective cytokine/endotoxin adsorption.- Onco-nephrology emergencies, immune-mediated renal injury, and drug-induced nephrotoxicity in intensive care.- Translational advances in regenerative medicine, stem cell therapy, and biomarkers guiding renal recovery.- Long-term renal trajectories, transition from AKI to chronic kidney disease, and post-critical care follow-up models. All manuscripts must have clear educational value and include a contextual literature review.
