Cross-Setting Dementia care Pathways in Older Adults Living with Dementia
Older adults living with dementia frequently present with complex clinical and care needs, including delirium superimposed on dementia, behavioural and psychological symptoms, functional decline, frailty, caregiver burden, and repeated transitions across care settings. Despite the high prevalence and major consequences of these conditions, evidence remains fragmented across memory clinics, hospitals, nursing homes, respite care, and community services. Non-pharmacological interventions are widely recommended, yet their implementation in real-world care for older adults remains inconsistent. This Research Topic aims to gather high-quality contributions on the assessment, prevention, and management of delirium and other complex care needs in older adults with dementia, with a particular focus on clinically grounded, cross-setting dementia care and person-centred approaches. We welcome research addressing occupational therapy, activity-based interventions, environmental adaptations, dementia-friendly hospital models, staff training, structured daily routines, caregiver-supported programs, and service models that improve outcomes in complex older adult patients. The topic is especially interested in studies that connect clinical phenomena such as delirium, psychomotor subtypes, neuropsychiatric symptoms, and functional impairment with practical models of care. Contributions from hospitals, memory clinics, nursing homes, Alzheimer special care units, respite care, and community programs are all in scope. We also encourage implementation studies on staff training and care models, feasibility studies, cohort studies, comparative effectiveness research, and reviews that clarify what works, for whom, and in which setting. This collection will help advance a more integrated and clinically useful evidence base for the care of older adults living with dementia, to ultimately bridge clinical geriatric practice and dementia care research with a focus on outcomes that matter in the real world.
