Deep Phenotyping HFpEF: Translating Insights From Experimental Models to Patient Subtypes
Heart failure with preserved ejection fraction (HFpEF) is a heterogeneous syndrome in which conventional clinical classifications often fail to reflect underlying biology, limiting the success of targeted therapies. Deep phenotyping integrates detailed clinical data with imaging, hemodynamics, and molecular profiling. This approach offers a path to define biologically coherent HFpEF subtypes and advance precision treatment. This Research Topic focuses on deep phenotyping of HFpEF across species, linking experimental models with deeply characterized human cohorts. It will highlight complementary approaches such as metabolism focused mouse models, hemodynamics focused pig models, and large national and international initiatives. This Research Topic aims to define mechanism based HFpEF subtypes using deep phenotyping, and to bridge experimental models and human cohorts by aligning model derived mechanisms with patient level phenotypes and clinical outcomes. It also seeks to support translational strategies for precision therapy, including subtype driven trial design and targeted interventions, with the overall goal of establishing a framework for HFpEF subtyping that is both biologically grounded and clinically actionable. We mainly welcome original research, reviews, mini reviews, perspectives, and methodological papers. Topics of interest include, but are not limited to:Experimental models and mechanisms:• Metabolism focused mouse models and hemodynamics focused pig models of HFpEF;• Mechanistic studies of metabolic, hemodynamic, structural, inflammatory, and fibrotic pathways;• Cross species studies linking experimental phenotypes to human HFpEF.Deep phenotyping in human cohorts:• Advanced imaging, invasive and non invasive hemodynamics, exercise testing, and digital biomarkers;• Multi omic profiling, including genomics, transcriptomics, proteomics, and metabolomics.Data integration, subtyping, and methods:• Machine learning and AI approaches for HFpEF subtype discovery;• Integrative multimodal analysis pipelines linking models and patient cohorts;• Comparative studies across cohorts or consortia to define reproducible subtypes.Translational and therapeutic implications:• Deep phenotyping for risk stratification and enrichment of clinical trials;• Mechanism based therapeutic strategies tailored to specific HFpEF subtypes;• Conceptual papers proposing frameworks for precision HFpEF care.
