Emerging Horizons in Transcatheter Mitral Valve Interventions: Innovation, Outcomes, and Clinical Translation

Transcatheter mitral valve interventions have become a cornerstone in the evolving management of mitral valve disease, driven by the growing population of patients unsuitable for conventional surgery. Despite major advances in transcatheter mitral valve repair (TMVr) and replacement (TMVR), challenges remain in balancing device performance, procedural precision, and long-term durability. Recent evidence reveals promising improvements in procedural safety and efficacy, supported by innovations in imaging, patient-tailored approaches, and post-procedural management. However, uncertainties persist regarding optimal patient selection, comparative outcomes between repair and replacement, device durability, and integration into standard care. As technological refinement and clinical experience expand, a systematic and collaborative evaluation of these therapies remains crucial to enhance patient outcomes and inform future guidelines. This Research Topic aims to consolidate the latest research, technological innovation, and clinical expertise in TMVr and TMVR. It seeks to explore key scientific and clinical questions, including how novel device designs influence hemodynamic performance, what imaging modalities best support accurate deployment, and which patient characteristics predict procedural success and favorable long-term outcomes. By providing a platform for multidisciplinary contributions, this Research Topic aims to generate evidence-based insights that guide clinical decision-making, foster iterative innovation, and refine best practices for minimally invasive mitral valve therapy. To gather further insights in the field of transcatheter mitral valve therapy and its clinical application, we welcome articles addressing, but not limited to, the following themes:o Advances in device engineering and design mechanisms for TMVr and TMVRo Strategies for patient selection, risk stratification, and clinical decision pathwayso Role of multimodal imaging for procedural planning and outcome assessmento Innovations in procedural techniques and peri-procedural managemento Clinical trial data and real-world evidence on safety, durability, and efficacyo Evaluation of multidisciplinary team models and patient-centered care pathwayso Long-term follow-up studies on valve function, quality of life, and survival**TE Dr. Muhammed Gerçek receives consulting fees from Edwards Lifesciences.
