Endoscopic Management for Obesity

Obesity is a chronic, relapsing, and multifactorial disease that represents a major global health challenge, contributing substantially to the burden of metabolic, cardiovascular, hepatobiliary, and gastrointestinal disorders. While lifestyle modification, pharmacotherapy, and bariatric surgery remain key pillars of treatment, endoscopic approaches have emerged as an increasingly important option in obesity management. These techniques offer minimally invasive alternatives and adjuncts that can help bridge the gap between medical and surgical therapy, while expanding access to more personalized care for a broad range of patients.Recent advances in endoscopic bariatric and metabolic therapies have widened the therapeutic landscape. These include intragastric balloons, endoscopic sleeve gastroplasty, aspiration therapy, duodenal and small bowel interventions, revision endoscopy after bariatric surgery, and combination strategies integrating endoscopy with anti-obesity medications and multidisciplinary care pathways. At the same time, important questions remain regarding:● Patient selection● Durability of weight loss● Metabolic outcomes● Procedural standardization● Adverse event management● Long-term follow-up● Real-world implementationThis Research Topic, hosted in the Endoscopy section of Frontiers in Gastroenterology, aims to provide a comprehensive overview of current progress and future directions in the endoscopic management of obesity. We welcome submissions addressing technical innovation, comparative effectiveness, mechanisms of action, safety and quality metrics, clinical outcomes, cost-effectiveness, and the integration of endoscopic therapies into obesity care pathways.Contributions exploring the role of endoscopy in the following areas are also encouraged:• Special populations• Revision procedures• Multidisciplinary treatment modelsBy bringing together original research, reviews, and expert perspectives, this collection seeks to advance the evidence base for endoscopic obesity therapies and support their optimal use in clinical practice.
