Endothelial Dysfunction, Vascular-Organ Crosstalk, and Therapeutic Targets in Cardiovascular Disease

Cardiovascular disease (CVD) remains a leading cause of morbidity and mortality worldwide. A growing body of evidence identifies endothelial dysfunction as a central driver of vascular pathology, promoting inflammation, oxidative stress, and impaired vascular tone. Beyond local effects, endothelial dysfunction participates in vascular-organ crosstalk, affecting the major organs like heart, kidneys, brain, lung and liver, thereby contributing to multi-organ complications. Despite advances in pharmacology, many mechanisms linking endothelial dysfunction to organ-level outcomes remain incompletely understood. Targeting these pathways offers promising opportunities for novel therapeutic interventions to prevent or reverse CVD progression. This Research Topic will focus on elucidating these mechanisms and highlighting translational strategies for restoring vascular health and organ function.Despite significant advances in cardiovascular medicine, CVD continues to cause high morbidity and mortality worldwide. A major contributor is endothelial dysfunction, which not only impairs vascular homeostasis but also mediates pathological vascular-organ crosstalk, leading to cardiac, renal, hepatic, pulmonary and neurovascular complications. Current therapies often address symptoms, such as high blood pressure, rather than the underlying molecular and cellular mechanisms driving endothelial injury and organ-level interactions. The goal of this Research Topic is to bring together research that elucidates the mechanistic pathways of endothelial dysfunction and inter-organ signaling in CVD, and to identify novel therapeutic targets for pharmacological intervention. By integrating basic science, translational studies, and clinical research, this collection aims to highlight strategies that can restore vascular health, prevent multi-organ damage, and improve health outcomes. Contributions may include experimental studies, reviews, and mechanistic insights that bridge molecular pathways with therapeutic prospects. This Research Topic invites contributions that explore the mechanisms, consequences, and therapeutic opportunities of endothelial dysfunction and vascular–organ crosstalk in CVD. Specific themes of interest include molecular and cellular pathways of endothelial injury, vascular inflammation, oxidative stress, and nitric oxide signaling; interactions between the vasculature and the heart, kidneys, brain, and other organs; identification of novel pharmacological targets; and translational studies linking mechanistic insights to therapeutic interventions. We welcome original research articles, reviews, mini-reviews, perspectives, and hypothesis-driven studies that provide mechanistic understanding or propose innovative strategies to restore vascular health and prevent multi-organ complications in CVD. Interdisciplinary studies that integrate basic, preclinical, and clinical research are particularly encouraged to highlight the translational relevance of these mechanisms for therapeutic development.
