Endovascular Neurosurgery at the Frontier: Emerging Techniques, Expanding Indications, and Applied Technologies
Endovascular techniques represent one of the most significant advances in modern medicine, particularly in the fields of Vascular Neurology and Neurosurgery. Notably, mechanical thrombectomy and endovascular aneurysm occlusion procedures have established their effectiveness and cost-efficiency in treating conditions like subarachnoid hemorrhage and ischemic stroke. However, in some medical areas, their adoption has been more reliant on indirect evidence and the allure of less invasive methods. Despite these constraints, the pace of endovascular innovation continues to enable the treatment of increasingly complex diseases, achieving favorable morbidity and mortality profiles. This optimistic perception has fueled their expansion beyond aneurysmal disease and ischemic stroke, into the whole spectrum of neurovascular diseases and, more recently in many other neurosurgical fields. Furthermore, these advances have enabled the development of hybrid procedures and could potentially redefine the way in which conventional neurosurgery has approached these pathological domains. Combined with the rising influence of artificial intelligence and robotics, these advances could profoundly reshape how Neurology, Neurosurgery, and Interventional Neuroradiology are practiced. This Research Topic aims to showcase the most recent advances, innovative techniques, and emerging trends in the treatment of both vascular and non-vascular diseases where endovascular approaches play a central role.A key focus is the development of hybrid techniques where endovascular and open surgical strategies intersect, thereby enhancing and extending the possibilities of each field. This integration offers a promising opportunity to rethink how neurosurgical practice is conceptualized and executed. An in-depth assessment of the current state of the art across these domains will help identify how endovascular and hybrid innovations may redefine standard indications while improving our current interventional treatments. We seek contributions that critically examine the challenges and opportunities arising from these transformations, while addressing the requirements for their safe and effective integration into clinical practice. Finally, the rapid advances in artificial intelligence and robotics add a further dimension, offering tools that can enhance clinical decision-making and broaden access to high-quality care, accelerating the evolution of both endovascular and hybrid neurosurgical practice. The scope of this Research Topic encompasses the applications, benefits, limitations, justifications, innovations, and future perspectives of endovascular therapies applied to non-vascular neurological and neurosurgical diseases. Additionally, it covers the development of novel endovascular and hybrid approaches for vascular conditions and explores the influence of emerging technologies, such as artificial intelligence and robotics, across these domains.Areas of interest include, but are not limited to:· Middle meningeal artery embolization for chronic subdural hematomas· Novel approaches to intracranial hypertension and tinnitus, such as venous stenting· Endovascular techniques applied to functional neurosurgery· Endovascular treatment of hydrocephalus· Hybrid techniques and hybrid operating rooms· Artificial intelligence and robotics applied to endovascular and hybrid therapies· Challenges and opportunities in the adoption of endovascular techniques for non-vascular neurological and neurosurgical diseasesWe welcome contributions in the form of Original Research, Case Reports, Methods, Reviews, Systematic Reviews, and Clinical Trials.
