Endurance and Extreme Condition Sports: Cardiovascular Mechanisms, Monitoring, and Risks

In recent years, there has been an explosive growth in participation in high-intensity endurance events, ranging from marathons and trail running to ultramarathons and high-altitude mountain sports. As both elite and amateur athletes push their physical limits, it is increasingly vital to understand the impact of these prolonged physiological stressors on the cardiovascular system. This Research Topic broadens the lens to capture the intersection of traditional sports cardiology and exercise physiology across all major endurance and extreme-environment disciplines. (Note: We specifically exclude short-burst or court-based sports, such as basketball or volleyball, to maintain a strict focus on prolonged high-intensity endurance.)We welcome papers that bridge the gap between healthy physiological adaptation and cardiac pathology. This collection is cross-listed between Frontiers in Cardiovascular Medicine and Frontiers in Physiology, encouraging multidisciplinary research.Themes of interest include, but are not limited to:- Cardiovascular adaptation and maladaptation in marathon, ultramarathon, and trail running athletes.- The physiological and cardiac impact of sports performed in extreme environmental conditions, including high altitude (hypoxia), deep-water and freediving (hyperbaric stress), aerospace environments, and extreme temperatures (heat and cold).- The physiological interplay between environmental stressors and prolonged exertion on cardiac function.- Advanced monitoring techniques for evaluating cardiac function and arrhythmias during prolonged high-intensity exercise in the field.- Risk stratification and prevention of sudden cardiac death in endurance athletes.- The clinical management of amateur and professional athletes engaging in extreme endurance sports.Appendix: We invite original research, systematic and narrative reviews, brief reports, clinical case studies, and innovative perspectives.
