Epidemiological Predictors, Nutritional Biomarkers, and Body Composition Dynamics in the Rapid Progression of Chronic Kidney Disease
Rapid progression of Chronic Kidney Disease (CKD) remains a critical global health challenge, leading to high morbidity, early mortality, and substantial healthcare costs. While classical risk factors like proteinuria and hypertension are well-documented, emerging evidence highlights the pivotal role of metabolic shifts, nutritional status, and body composition dynamics in accelerating renal decline. Conditions such as Protein-Energy Wasting (PEW), fluid overload, sarcopenia, and sarcopenic obesity frequently manifest before end-stage kidney disease, yet their predictive value in early stage CKD trajectory remains underutilized. Furthermore, identifying these dynamics requires a lifespan perspective that encompasses both pediatric and adult populations. Integrating advanced clinical epidemiology with novel nutritional biomarkers and functional body composition metrics (such as bioelectrical impedance vector analysis) offers a transformative avenue for early risk stratification and targeted interventions. The primary objective of this Research Topic is to bridge the gap between nutritional epidemiology, body composition monitoring, and clinical nephrology to better predict, understand, and attenuate rapid CKD progression across all ages. By focusing on non-traditional predictors and subtle metabolic alterations, this collection aims to establish novel diagnostic frameworks and intervention targets. We seek to highlight high-quality research demonstrating how precise evaluation of body composition, metabolic biomarkers, and nutritional status can inform early decision-making, optimize therapy, and slow renal decline. Ultimately, this topic intends to unite pediatric and adult nephrologists, clinical nutritionists, and epidemiologists to redefine risk stratification models in CKD, moving from reactive management toward proactive, individualized metabolic and clinical interventions that preserve kidney function and improve patient outcomes globally. We welcome original research articles, systematic reviews, clinical trials, observational cohorts, and brief research reports focusing on pediatric or adult populations with CKD. Themes of interest include, but are not limited to:- Predictive Models & Risk Assessment: Epidemiological studies and predictive algorithms identifying risk factors for rapid CKD trajectory.- Nutritional Biomarkers & PEW: Novel biomarkers, metabolic markers, and clinical scores evaluating nutritional deterioration and its direct impact on renal survival.- Body Composition Dynamics: Studies using bioimpedance (BIVA/BIS), DXA, or functional metrics to explore sarcopenia, fluid overload, and sarcopenic obesity as early predictors of decline.- Lifespan Perspectives: Pediatric-specific cohorts examining developmental, nutritional, and etiology-driven predictors of rapid progression.
