Evolving Horizons in Cardiac Intensive Care: Pharmacological, Technological, and Ethical Dimensions

Cardiology intensive care represents a dynamic and evolving field at the intersection of critical care medicine and cardiovascular science. The management of critically ill cardiac patients, who often exhibit intricate pathophysiological profiles, requires sophisticated diagnostic, pharmacological, technological, and ethical approaches. Recent advancements in mechanical circulatory support, real-time monitoring, precision pharmacotherapy, and digital health technologies have revolutionized the treatment of acute cardiac conditions such as cardiogenic shock, acute heart failure, arrhythmias, and post-cardiac arrest syndromes. Alongside these technological and therapeutic advancements, clinicians are increasingly confronting ethical challenges related to resource allocation, end-of-life care, and shared decision-making in high-pressure environments. At the same time, an ageing and increasingly comorbid patient population is raising new questions about frailty, sex and gender differences in presentation and outcomes, and how care teams are staffed and trained to meet rising CICU acuity. This Research Topic seeks to encompass the modern landscape of cardiology intensive care by soliciting contributions from multidisciplinary teams involved in clinical practice, research, ethics, and healthcare innovation. We encourage submissions of original research articles, clinical trials, systematic reviews, case series, technology reports, and perspectives that tackle both novel developments and ongoing challenges in the management of critically ill cardiac patients. To gather further insights into the complex domain of cardiology intensive care, we welcome articles addressing, but not limited to, the following themes:• Pharmacological Innovations: Exploration of new therapeutic agents, dosing strategies, and precision medicine approaches for acute cardiac syndromes and hemodynamic stabilization• Technological Progress: Advances in mechanical circulatory support technologies such as ECMO and VADs, innovations in digital and wearable health technologies, applications of remote ICU monitoring, and artificial intelligence — including AI-based risk prediction and early-warning models for the cardiac ICU• Clinical Outcomes and Protocols: Development of management algorithms, prognostic models, biomarker-guided therapies, neuroprognostication after cardiac arrest, and evaluation of real-world outcomes in cardiac intensive care settings• Vulnerable and Under-Studied Populations: Research into frailty and multimorbidity in the cardiac ICU, sex and gender differences in presentation, treatment response and outcomes, and post-ICU recovery, rehabilitation, and long-term quality of life• Ethical and Organizational Considerations: Research into resource allocation strategies, end-of-life decision-making processes, communication, and psychosocial issues within the cardiac ICU environment• Education and Team Dynamics: Studies of interdisciplinary training programs, team-based care models, CICU staffing and workforce models, and leadership strategies within cardiology intensive care units. The overarching aim is to stimulate a comprehensive and forward-looking dialogue to promote improvements in patient outcomes, safety, and ethical standards in cardiology intensive care across the globe.
