Fertility Preservation and Pregnancy Planning in the Era of Novel Therapies: Navigating Oncological and Biological Treatments in Reproductive-Age Women

Reproductive endocrinology has entered a new era shaped by the rapid growth of targeted oncological agents and biological therapies. These treatments have transformed outcomes in cancer and chronic inflammatory diseases but introduced new questions about their effects on ovarian function, reproductive hormones, and long-term fertility potential. While the gonadotoxic risks of chemotherapy and radiotherapy are well characterised, far less is known about how CDK4/6 inhibitors, tyrosine kinase inhibitors, PARP inhibitors, immune checkpoint inhibitors, and biologics impact the hypothalamic–pituitary–ovarian axis, gametogenesis, and the possibility of achieving a healthy pregnancy after treatment. Recent studies have begun mapping these effects through clinical observation, biomarkers such as anti Müllerian hormone (AMH), and histological analyses of ovarian tissue, yet findings remain fragmented and mechanisms uncertain. The field now faces the challenge of bridging pharmacologic discovery with reproductive endocrinology to safeguard fertility and hormonal health in affected women. This Research Topic aims to elucidate the endocrine and reproductive consequences of targeted and biological therapies in women of reproductive age, integrating mechanistic, translational, and clinical perspectives. It seeks to define how emerging drugs influence ovarian reserve, folliculogenesis, steroidogenesis, and the broader endocrine milieu, and how these translate into fertility potential, pregnancy outcomes, and long-term reproductive health. The overarching goal is to inform strategies for fertility preservation, counselling, and post treatment conception, fostering multidisciplinary collaboration across oncology, immunology, and reproductive medicine. To gather further insights within the boundaries of reproductive endocrinology and therapy related gonadotoxicity, we welcome articles addressing, but not limited to, the following themes:o Gonadotoxic profiling of targeted and biological agents across malignancies and chronic inflammatory conditionso Mechanistic studies on ovarian reserve, follicular dynamics, and hypothalamic–pituitary–ovarian function under novel therapieso Optimization of ovarian stimulation and fertility preservation protocols in oncofertility and chronic disease settingso Assessment of male reproductive function in the context of systemic targeted and immunomodulatory treatmentso Pregnancy outcomes and obstetric safety following novel therapeutics and fertility interventionso Endocrine adjuvant therapies and their influence on reproductive recovery and conception planning Topic editors: Giuseppe Gabriele Iorio, Claudia Massarotti, Carlo Alviggi Source: https://www.frontiersin.org/research-topics/82032/fertility-preservation-and-pregnancy-planning-in-the-era-of-novel-therapies-navigating-oncological-and-biological-treatments-in-reproductive-age-women
