Fragility Fractures: A Multidisciplinary Frontier in Diagnosis, Treatment, and Prevention

Population aging is a critical challenge in contemporary healthcare, giving rise to a host of age-associated diseases, among which fragility fractures are particularly concerning. These injuries, often emerging from minimal trauma, highlight not only local musculoskeletal compromise but also systemic bone fragility linked to underlying osteoporosis. Recent studies have emphasized the soaring incidence of fragility fractures and their profound impact on morbidity, mortality, and overall healthcare costs. Fragility fractures present a diverse spectrum of clinical demands, ranging from minor injuries to ones having a high surgical impact. Despite advances in surgical techniques for managing such fractures, gaps remain in holistic management of these patients and secondary prevention. Growing evidence suggests that failure to identify fragility fractures as sentinel events of osteoporosis perpetuates a cycle of recurrent fractures and suboptimal outcomes. The objective is to move beyond the traditional “fracture-to-treat” paradigm, shifting toward a comprehensive “patient-to-treat” strategy. This Research Topic aims to serve as a multidisciplinary platform uniting orthopedic surgeons, internists, endocrinologists, rheumatologists, anesthesiologists, epidemiologists, and rehabilitation specialists to address fragility fractures from a patient-centered perspective. Key questions include: How can early recognition and intervention be optimized to prevent re-fractures? What are the most effective models for integrating pharmacological, surgical, and rehabilitative care? Can coordinated Fracture Liaison Services demonstrably reduce costs and improve long-term outcomes? The scope of this Research Topic encompasses the full continuum of care for fragility fractures, while prioritizing interdisciplinary approaches and focusing on osteoporosis as an underlying systemic concern. It explicitly excludes research limited solely to technical descriptions of surgical devices or procedures without broader clinical context. To gather further insights into the multifaceted landscape of fragility fractures, we welcome contributions addressing, but not limited to, the following themes:• Surgical innovations in management of osteoporotic fractures• Comparative outcomes of conservative versus surgical treatment strategies• Early diagnosis and metabolic optimization of osteoporosis in fracture patients• Multidisciplinary pathways for secondary fracture prevention• Perioperative care models and anesthetic considerations for fragile patients• Cost-effectiveness and impact of Fracture Liaison Services (FLS)• Rehabilitation protocols tailored to frail and elderly individualsWe invite original research articles, reviews, clinical guidelines, case studies, and perspectives related to the above themes and the overarching goal of improving outcomes for patients with fragility fractures.Topic Editor Prof. Angela Notarnicola has received grant from Italfarmaco, UCB, Amgen, Bruno, EG, and Stada for scientific collaborations.
