From Scene to System: Connecting Prehospital, Clinical and Registry Data in Trauma Care

Major trauma remains a leading cause of death and long-term disability worldwide. Although trauma systems have evolved through the establishment of trauma centers, regional trauma networks, standardized treatment pathways, and national trauma registries, important gaps in data integration persist. Most trauma registries primarily focus on in-hospital treatment and outcomes, while critical information generated before hospital arrival is often unavailable or only partially documented. This Research Topic aims to explore how objective data generated throughout the trauma care continuum can be integrated to improve clinical decision-making, trauma system performance, quality assurance, and registry-based research. Particular attention will be given to the transition from isolated datasets toward connected trauma information ecosystems that link prehospital, clinical, and registry data. The collection seeks to identify innovative approaches for integrating data from incident detection, emergency response, hospital treatment, rehabilitation, long-term outcomes, and trauma registries. Special emphasis will be placed on the use of emerging technologies such as AACN and eCall systems, vehicle telematics, wearable sensors, electronic documentation systems, artificial intelligence, and clinical decision-support tools.Furthermore, the Research Topic aims to address scientific, technical, organizational, ethical, and legal challenges related to data integration, including interoperability, data quality, validation, privacy, data protection, and implementation strategies. By fostering interdisciplinary collaboration, the issue seeks to advance the development of internationally connected trauma systems and support future standards for integrated trauma data infrastructures. We welcome original research articles, reviews, perspectives, methodological studies, and implementation reports addressing the integration of prehospital, clinical, and registry data within trauma systems. Topics of interest include, but are not limited to, AACN and eCall technologies, vehicle telematics, crash reconstruction data, wearable technologies, dispatch center information, emergency medical services documentation, electronic health records, trauma registries, data linkage methodologies, artificial intelligence, and clinical decision-support systems. Submissions focusing on major trauma, polytrauma, trauma team activation, destination decision-making, injury severity prediction, trauma system evaluation, quality assurance, outcome assessment, and guideline development are particularly encouraged. Contributions addressing interoperability, data governance, privacy and data protection, standardization, international harmonization, and implementation challenges are equally welcome. This Research Topic aims to bring together clinicians, trauma surgeons, emergency physicians, EMS professionals, registry scientists, engineers, epidemiologists, health informaticians, and policymakers to advance integrated trauma data ecosystems and improve patient care worldwide.
