Future Directions in Genitourinary Oncology
Genitourinary oncology is being reshaped by advances in molecular biology, precision medicine, artificial intelligence, imaging, and therapeutic innovation. Although genomic profiling, biomarker-guided treatment, immunotherapy, radioligand therapy, antibody–drug conjugates, and minimally invasive interventions are expanding the options available for patients with prostate, bladder, kidney, testicular, and other genitourinary malignancies, substantial challenges remain. These include improving early detection and risk stratification, identifying patients most likely to benefit from specific treatments, predicting and overcoming therapeutic resistance, and ensuring that scientific discoveries can be implemented equitably in routine care. Recent studies have demonstrated the potential of liquid biopsies, circulating biomarkers, radiomics, digital pathology, and machine learning to support diagnosis, prognostication, treatment selection, and disease monitoring. At the same time, advances in immuno-oncology, targeted therapies, radioligand approaches, surgery, and radiation are generating new opportunities for personalized treatment. However, the clinical validation, reproducibility, interpretability, and integration of these innovations remain incomplete. Further investigation is needed to bridge laboratory discoveries and clinical practice, refine multidisciplinary care, and establish evidence-based approaches that improve outcomes and quality of life across diverse patient populations. This Research Topic aims to highlight emerging concepts and future directions in genitourinary oncology across basic, translational, and clinical research. It will examine how novel biomarkers, advanced imaging, computational technologies, therapeutic strategies, and innovative models of care can improve diagnosis, risk assessment, treatment response prediction, resistance management, and patient-centered outcomes. The Research Topic will also address the evidence required to translate promising discoveries into clinical practice, including the development of robust clinical trials, real-world evidence, implementation strategies, and approaches that promote health equity. To gather further insights into emerging diagnostic, therapeutic, translational, and patient-centered strategies across genitourinary malignancies, we welcome articles addressing, but not limited to, the following themes:Molecular diagnostics, genomics, biomarker discovery, and precision oncologyAdvanced imaging and image-guided diagnosis or treatmentArtificial intelligence, machine learning, radiomics, and digital pathologyLiquid biopsy and circulating tumor biomarkersImmunotherapy, targeted therapy, antibody–drug conjugates, and radioligand therapyMechanisms of treatment resistance and strategies to overcome resistanceTranslational research bridging laboratory discoveries and clinical practiceInnovative clinical trial designs and real-world evidenceAdvances in surgical, radiation, systemic, and multidisciplinary managementSurvivorship, quality of life, and patient-centered careHealth equity, implementation science, and digital health in genitourinary oncologyAppendix: We welcome Original Research, Review, Mini Review, and Perspective articles.
