Genetics of Multiple Myeloma: from Diagnosis to Treatment

Multiple Myeloma (MM) research is at the forefront of personalized medicine, driven by rapid advancements in laboratory genomics and molecular diagnostics. Genomic studies have uncovered a spectrum of cytogenetic and molecular alterations that significantly impact disease initiation, progression, and therapeutic outcomes. These insights have already shifted clinical paradigms—customizing risk stratification, enhancing prognostic modeling, and influencing therapy decisions. Recent developments such as single-cell multi-omics and minimal residual disease (MRD) assessment at the molecular level have sharpened our ability to map clonal heterogeneity and monitor treatment response. Furthermore, innovative immunotherapies like CAR-T cells and bispecific antibodies are increasingly designed to match individual genetic and antigenic profiles. However, challenges remain in standardizing genomic testing, interpreting complex data, and ensuring equitable access to precision strategies. This Research Topic aims to explore and consolidate the latest evidence surrounding genetics-informed risk stratification and precision therapies in multiple myeloma. We seek to address key questions, including how novel genetic findings can further refine risk categories, the implications of clonal evolution on therapeutic resistance, and methods to successfully translate laboratory advances into real-world clinical practice. By gathering original research, comprehensive reviews, and expert perspectives, the focus will be on bridging laboratory discoveries with clinical applications to improve survival outcomes for MM patients. To gather further insights in the integration of genomics into MM risk assessment, treatment, and monitoring, we welcome articles that address, but are not limited to, the following themes:• The current genomic landscape and practical approaches to risk stratification in MM• Clonal heterogeneity, clonal evolution, and disease progression—mechanisms, detection, and impact on therapy• Genetics-driven therapeutic selection and the development of targeted strategies, including immunotherapy• MRD monitoring and precision assessment utilizing advanced molecular techniques, such as liquid biopsy• Opportunities and challenges in embedding genomics into routine MM care, with attention to workflow integration and policy implications We encourage the submission of original and review articles, technical reviews, case reports/series, research reports, and editorial or opinion pieces on these and related topics.
