Gynecologic Cancer Surgery: Rethinking the Perioperative Journey

Surgical management remains a cornerstone in the treatment of gynecologic malignancies, including ovarian, endometrial, cervical, vulvar, and vaginal cancers. Over the past decades, significant advances in surgical techniques, anesthetic management, and perioperative care have improved patient outcomes and reduced postoperative morbidity. The introduction of minimally invasive surgery, enhanced recovery after surgery (ERAS) protocols, optimized perioperative monitoring, and multidisciplinary perioperative planning has transformed the management of patients undergoing complex oncologic procedures. Despite these improvements, perioperative complications, delayed recovery, and variability in clinical practice remain important challenges in gynecologic oncology. A comprehensive understanding of perioperative risk assessment, optimization strategies, and postoperative care pathways is essential to enhance surgical outcomes and improve quality of life for patients undergoing treatment for gynecologic cancers. This Research Topic aims to explore current advances and emerging strategies in the perioperative management of patients undergoing surgery for gynecologic malignancies. As surgical procedures for gynecologic cancers can be extensive and complex, optimizing perioperative care is essential for improving patient safety, reducing complications, and enhancing recovery.Recent developments in preoperative assessment, patient optimization, anesthetic techniques, intraoperative management, and postoperative care have significantly influenced outcomes in gynecologic oncology surgery. Enhanced recovery pathways, improved perioperative monitoring, and multidisciplinary collaboration between surgeons, anesthesiologists, and perioperative care teams have contributed to more efficient and patient-centered care. This collection aims to bring together clinical and translational research addressing perioperative risk stratification, evidence-based perioperative protocols, complication prevention, and strategies to improve recovery and long-term outcomes. By highlighting innovative approaches and current challenges in perioperative care, this Research Topic seeks to support the development of standardized and evidence-based perioperative management strategies in gynecologic oncology. This Research Topic welcomes contributions addressing perioperative care and surgical management in patients with gynecologic malignancies. Submissions may include clinical studies, translational research, systematic reviews, and expert perspectives that advance knowledge in perioperative optimization and surgical outcomes.Topics of interest include, but are not limited to:• Preoperative risk assessment and patient optimization in gynecologic oncology• Enhanced Recovery After Surgery (ERAS) protocols in gynecologic cancer surgery• Anesthetic considerations and perioperative monitoring• Prevention and management of perioperative complications• Minimally invasive and robotic surgery in gynecologic oncology• Perioperative outcomes and quality improvement strategies• Multidisciplinary perioperative care pathways• Postoperative recovery, rehabilitation, and patient-centered outcomesWe welcome Original Research, Reviews, Systematic Reviews, Clinical Studies, and Perspectives related to perioperative management in gynecologic oncology. Please note that manuscripts consisting solely of bioinformatics or computational analysis of public omics databases that are not supplemented by relevant functional validation (clinical cohort or biological validation in vitro or in vivo) are out of scope for this Research Topic.Significant contributions have been made by Topic Coordinator, Maria Fanaki, in the assembling, management, and completion of this topic.
