Helicobacter pylori in Children in the Era of Antimicrobial Resistance

Helicobacter pylori infection remains an important clinical challenge in children. Its prevalence varies considerably according to socioeconomic conditions, hygiene, geography and regional patterns of transmission, with a particularly substantial burden in high-prevalence settings. Increasing resistance to clarithromycin and metronidazole has reduced the effectiveness of conventional triple therapy and strengthened the need for evidence-based, locally informed treatment strategies. Current ESPGHAN/NASPGHAN guidance has therefore placed greater emphasis on appropriate indications for testing, confirmation of infection, antimicrobial susceptibility and individualized eradication therapy. H. pylori may also be detected during upper gastrointestinal endoscopy performed for other indications, including coeliac disease, inflammatory bowel disease and eosinophilic oesophagitis. These situations raise important questions about clinical significance, treatment decisions and management in children with comorbidities or drug allergies. This Research Topic aims to bring together clinical and translational research on the epidemiology, diagnosis, treatment and prevention of H. pylori infection in children in the context of antimicrobial resistance. We welcome studies examining changing patterns of transmission, appropriate testing and treatment strategies, and the implementation of ESPGHAN/NASPGHAN recommendations in different healthcare settings. Particular interest will be given to non-invasive diagnostic approaches, molecular methods for detecting antimicrobial resistance, susceptibility-guided treatment, tailored eradication regimens, and strategies for managing treatment failure and reinfection. Contributions addressing H. pylori infection in children with coeliac disease, inflammatory bowel disease, eosinophilic oesophagitis, other gastrointestinal conditions, or antimicrobial and proton-pump-inhibitor allergies are also encouraged. The Research Topic will provide a multidisciplinary forum connecting pediatric gastroenterology, microbiology, infectious diseases, pathology and public health. We welcome articles addressing, but not limited to, the following themes:• Changing epidemiology and transmission of H. pylori infection in children• Application of ESPGHAN/NASPGHAN recommendations in clinical practice• Non-invasive diagnostic strategies and molecular resistance testing• Antimicrobial susceptibility-guided and individualized eradication therapy• Management of treatment failure, persistent infection and reinfection• H. pylori infection in children with coeliac disease, inflammatory bowel disease and eosinophilic oesophagitis• Eradication strategies for children with antimicrobial or proton-pump-inhibitor allergies• Long-term outcomes and gastric cancer prevention in high-risk families• Effects of eradication therapy on the gut microbiome• Antimicrobial stewardship and regional approaches to treatmentWe welcome original research, systematic and narrative reviews, mini-reviews, methods papers, perspectives, clinical practice articles and case reports that provide clinically relevant or translational insights Topic editors: Yasin Sahin, Eylem Sevinc, Nafiye Urganci Source: https://www.frontiersin.org/research-topics/86505/helicobacter-pylori-in-children-in-the-era-of-antimicrobial-resistance
