Hepatobiliary Surgery Guidance: Integrating Fluorescence, 3D Planning, and Advanced Imaging for Precision Outcomes
Modern hepatobiliary (HB) surgery is undergoing a paradigm shift, driven by the integration of sophisticated intraoperative guidance technologies. As surgical complexity increases, the reliance on traditional anatomical landmarks is being supplemented by a "digital surgical environment." This Research Topic explores the cutting-edge application of fluorescence imaging (ICG), 3D patient-specific modeling, and real-time navigation systems—tools that are redefining precision in the operating theater. While these technologies offer transformative potential for tumor localization, vascular mapping, and biliary anatomy delineation, their routine clinical adoption remains heterogeneous. Surgeons face a dual challenge: mastering the steep learning curve associated with these digital adjuncts while navigating the lack of standardized protocols and high implementation costs. The objective of this collection is to bridge the gap between technological innovation and evidence-based clinical benefit, fostering a consensus on how these tools can best enhance patient safety and oncological outcomes. This Research Topic aims to assemble a comprehensive collection of high-quality research that critically evaluates intraoperative guidance methods in HB surgery. We seek to highlight how fluorescence, 3D planning, and navigation systems directly influence intraoperative decision-making, surgical precision, and postoperative recovery. Our goal is to provide a platform for studies that demonstrate the tangible value of these technologies in reducing morbidity and optimizing resection margins, especially in the context of minimally invasive and robotic platforms. We welcome original research, systematic reviews, technical notes, and perspectives that address the following themes (but are not limited to):• Clinical Efficacy: Quantitative and qualitative analyses of fluorescence-guided (e.g., ICG) liver and biliary procedures;• 3D Innovations: The role of 3D virtual planning and 3D printing in preoperative strategy and real-time intraoperative guidance;• Navigation & Augmented Reality: Advances in intraoperative navigation, sensor-based tracking, and the integration of AI-assisted imaging;• Surgical Precision: Impact of guidance tools on lesion localization, R0 resection rates, and the preservation of critical structures;• Barriers to Implementation: Studies addressing learning curves, cost-benefit analyses, and the development of standardized protocols;• Technology Integration: Synergies between guidance tools and robotic or laparoscopic HB surgery;• Decision-Making: How real-time data flow alters the surgeon’s strategy and enhances patient safety. We welcome, but are not limited to, original research articles, clinical studies, systematic reviews, technical notes, case series, and perspectives.
