Holistic Approach and Precision Monitoring in Immune-metabolic Diseases in Childhood

Immune-metabolic diseases, such as diabetes mellitus and obesity, are presenting an increasing burden in pediatric populations worldwide. The interplay between immune system dysfunction and metabolic dysregulation results in chronic low-grade inflammation and progressive tissue damage, driving a multitude of adverse clinical outcomes. Novel research has highlighted that beyond genetics and classical risk factors, a complex web of environmental influences and immune-metabolic crosstalk underpins the rising incidence of these disorders in children. While recent years have seen technological and therapeutic advances—such as continuous glucose monitoring, hybrid closed-loop systems for diabetes, and GLP-1 receptor agonists in obesity—persistent gaps remain in our understanding of pathophysiology, optimal monitoring strategies, and the effective integration of emerging biomarkers and individualized therapies in real-life practice. The ongoing challenge is how to best apply these multidisciplinary and holistic approaches to achieve improved prognosis and quality of life. This Research Topic aims to explore the rapidly evolving landscape of monitoring and treatment in childhood immune-metabolic diseases by supporting the integration of holistic and precision approaches. Its objectives include exploring the value of new molecular markers and monitoring technologies for guiding diagnosis, risk assessment, and therapy; evaluating the influence of environmental and lifestyle factors on disease course and metabolic compensation; and appraising the safety and efficacy of innovations in treatment. By drawing on current evidence and encouraging multidisciplinary perspectives, this topic seeks to clarify how emerging knowledge can be harnessed to advance personalized medicine, improve patient outcomes, and inform future guidelines for pediatric care. The boundaries of this Research Topic are centered on immune-metabolic disorders manifesting in childhood, with a particular focus on integrating emerging technologies, biomarkers, and therapies within a holistic care framework. Submissions must address pediatric populations and relate directly to immune-metabolic disease monitoring, management, or pathophysiology. To gather further insights in this domain, we welcome articles addressing, but not limited to, the following themes:•Continuous glucose monitoring and hybrid closed loop systems in the management of type 1 diabetes mellitus•Pathways and mechanisms underpinning insulin resistance and type 2 diabetes in childhood•Advances in innovative biomarkers and nutritional status assessment for children with diabetes and obesity•Novel possibilities and challenges in therapeutic approaches for pediatric immune-metabolic diseases•Multidisciplinary and family-based issues in the comprehensive management of affected childrenThis Research Topic welcomes Original Research, Systematic Reviews, Narrative Reviews, and Clinical Studies that further our understanding, monitoring, and treatment of pediatric immune-metabolic diseases.
