Implementing Contemporary Cardiorenal Guidelines: From Evidence and Risk Stratification to Multidisciplinary Patient Care
The interplay between cardiac and renal dysfunction, commonly referred to as cardiorenal syndrome, presents considerable clinical challenges. The recent publication of updated cardiorenal, cardiovascular–kidney, and cardio-kidney-metabolic guidelines introduces novel recommendations for diagnosis, risk stratification, treatment, and multidisciplinary management, aiming to improve patient outcomes. This Research Topic seeks to explore the translation of these guidelines into clinical practice, identify potential barriers to their adoption, and discuss innovations that could enhance patient care across diverse healthcare settings. The Research Topic will focus on how contemporary recommendations can be implemented, evaluated, and adapted in routine cardiovascular, renal, internal medicine, and primary-care practice. Contributions on pathophysiology, biomarkers, diagnostic tools, and therapeutic innovations are encouraged where they directly support the interpretation or implementation of current guideline recommendations. The primary aim of this Research Topic is to provide a comprehensive platform for the discussion and critical appraisal of new cardiorenal guidelines. We aim to gather original research, reviews, and expert opinions that highlight the latest evidence, address controversies, evaluate guideline-concordant care, and propose practical solutions for implementing guideline recommendations across diverse healthcare settings. The Research Topic will also examine how guideline adoption can be measured and whether implementation improves clinical processes, treatment access, patient-centred care, and patient outcomes. We particularly welcome work addressing multidisciplinary care pathways, health-system variation, implementation barriers, and equity in access to recommended cardiorenal treatments. We welcome Original Research Articles, Systematic Reviews, Clinical Trial Reports, Policy and Practice Reviews, Methods and Study Protocols focused on guideline implementation or evaluation, and Opinion, Perspective, and General Commentary pieces.Key Themes Include:Pathophysiological insights into cardiorenal interactions where they inform current guideline recommendationsDiagnostic advancements and biomarkers for guideline-based diagnosis, risk stratification, treatment selection, or monitoringTherapeutic innovations, including pharmacological and device-based interventions, in the context of contemporary guideline recommendationsRisk stratification and patient-centred care approachesGuideline-concordant treatment pathways and clinical decision-makingHealth-system challenges and practical solutions for guideline implementationMultidisciplinary collaboration among cardiologists, nephrologists, internists, primary-care providers, pharmacists, and allied health professionalsDifferences in guideline adoption across regions, healthcare systems, and resource settingsTreatment access, equity, affordability, and barriers to recommended careDigital health, clinical decision-support tools, and artificial intelligence that support guideline-concordant cardiorenal careMeasures of implementation success, including adherence, care processes, treatment uptake, safety, quality of care, and patient outcomesImpact of guideline adoption on hospitalisation, mortality, quality of life, disease progression, and other patient-centred outcomesSubmissions should explicitly evaluate, compare, implement, or challenge current cardiorenal, cardiovascular–kidney, or cardio-kidney-metabolic guideline recommendations. Studies focused solely on cardiorenal mechanisms, biomarkers, therapies, or prediction models without a clear connection to guideline translation, clinical implementation, or evaluation will be considered outside the primary focus of this Research Topic.
