Inflammation Across Obstetrics and Gynecology: From Mechanisms to Outcomes

Inflammation is a fundamental process throughout reproductive and women's health, supporting ovulation, menstruation, implantation, placentation, parturition, postpartum repair, and reproductive aging. When dysregulated, inflammatory responses contribute to infertility, pregnancy complications, infection, benign gynecologic disorders, surgical morbidity, malignant transformation, cancer progression, and treatment resistance. These responses may be local or systemic, sterile or infection-associated, and acute or chronic, while being shaped by endocrine, metabolic, microbial, vascular, stromal, genetic, environmental, and therapeutic factors. Despite substantial advances in immunology, molecular biology, and clinical research, evidence remains fragmented across organs, diseases, life stages, and disciplines. An integrated, disease-agnostic framework is needed to distinguish shared from context-specific inflammatory mechanisms and to clarify their relevance to reproductive medicine, obstetrics, benign gynecology, maternal-fetal medicine, gynecologic oncology, and long-term women’s health. Even though recent progress offers new opportunities, clinically validated biomarkers, reproducible endotypes, and effective strategies for prevention and treatment remain limited. This Research Topic aims to establish inflammation as a cross-cutting biological and clinical framework across the full spectrum of obstetrics and gynecology. It will examine how inflammatory responses are initiated, regulated, compartmentalized, sustained, and resolved, and how they influence prevention, diagnosis, prognosis, treatment selection, treatment response, recovery, and short- and long-term health outcomes. The Research Topic will connect basic, translational, clinical, computational, and population-level research to identify shared inflammatory mechanisms, clinically meaningful biomarkers and endotypes, and actionable therapeutic targets across physiological reproduction, fertility, pregnancy, benign gynecologic conditions, infection, surgery, reproductive aging, premalignant lesions, and gynecologic cancers. To gather further insights into inflammatory mechanisms and their clinical translation across reproductive and women’s health, we welcome articles addressing, but not limited to, the following themes:o Physiological and pathological inflammation in reproductive biology, fertility, assisted reproduction, pregnancy, placentation, labor, postpartum recovery, and reproductive aging;o Inflammatory mechanisms, biomarkers, imaging, risk prediction, treatment response, and outcomes in benign gynecology, pelvic disorders, infection, and gynecologic surgery;o Tumor-associated inflammation, immune–stromal interactions, prevention, diagnosis, prognosis, therapeutic resistance, treatment toxicity, survivorship, and outcomes in premalignant lesions and gynecologic cancers;o Local–systemic inflammatory crosstalk involving endocrine, metabolic, vascular, microbial, autoimmune, environmental, and lifestyle factors;o Applications of omics, single-cell and spatial technologies, artificial intelligence, computational models, organoids, biobanks, and biomarker validation;o Anti-inflammatory, immunomodulatory, hormonal, nutritional, lifestyle, surgical, and implementation strategies;o Patient-reported, reproductive, maternal, offspring, and long-term health outcomes associated with inflammation.
