Innovations and Outcomes in Breast Reconstruction: Autologous Tissue and Implant Approaches

Breast reconstruction is a dynamic and integral field in the multidisciplinary management of breast cancer patients following mastectomy or breast-conserving surgery. As survival rates improve and patient expectations rise, the focus on reconstructive outcomes (including physical form, psychological well-being, and long-term satisfaction) has never been greater. Contemporary practice involves two leading approaches: autologous reconstruction, which uses the patient’s tissue, and implant-based reconstruction, both offering distinctive benefits and unique challenges. While advances in microsurgery and implant technology have broadened the reconstructive landscape, critical questions persist regarding the optimization of techniques, patient selection, management of complications, and the impact of adjuvant therapies such as radiotherapy. Recent studies underscore evolving trends—such as the shift toward prepectoral implants and the refinement of free flap harvests—as well as the rising importance of patient-centered outcome measures. Nevertheless, ongoing debates center around complication rates, long-term safety, aesthetic results, and shared decision-making.This Research Topic aims to showcase the latest technical, clinical, and patient-centered advances in autologous and implant-based breast reconstruction. The main objectives are to compare outcomes and complications associated with each method; evaluate emerging approaches, including hybrid, staged, and combined techniques; and explore best practices for optimizing perioperative protocols and patient satisfaction. Further, we seek to understand the determinants of patient preference and quality of life, characterize strategies for challenging cases (e.g., post-radiotherapy, infection, or revision surgeries), and critically assess the role of innovation in guiding personalized treatment planning.By focusing on both immediate and delayed reconstruction pathways, this Research Topic encompasses the full spectrum of interventions while emphasizing evidence-based and patient-tailored strategies. Submissions may address adult and adolescent populations but should primarily relate to reconstructive surgery after cancer therapy. To gather further insights in this broad and evolving area, we welcome articles addressing, but not limited to, the following themes:o Surgical innovations in autologous flap and implant-based reconstructiono Comparative analyses of prepectoral versus subpectoral implant placemento Role and outcomes of fat grafting and acellular dermal matriceso Reconstruction in the context of radiation therapy and challenging clinical scenarioso Management of complications (e.g., infection, capsular contracture, flap failure)o Psychosocial and patient-reported outcomes in reconstructive surgeryo Strategies for revision and secondary reconstructiono Perioperative pathways and enhanced recovery protocolso Advances in patient selection, counseling, and shared decision-making Topic editors: Gennaro D'Orsi, Matteo Atzeni Source: https://www.frontiersin.org/research-topics/78071/innovations-and-outcomes-in-breast-reconstruction-autologous-tissue-and-implant-approaches
