Innovations and outcomes in minimally invasive and hybrid procedures for adult valvular interventions

Structural Interventional Cardiology is experiencing a paradigm shift, driven by the rapid development of minimally invasive and hybrid approaches for treating adult valvular heart disease. Traditional open-heart surgery is increasingly being supplemented—if not replaced—by advanced transcatheter devices and novel surgical techniques that offer patients improved safety profiles and quicker recoveries. However, this field remains in flux, with ongoing debates about optimal indications, procedural limitations, and the long-term status of these techniques as potential gold standards in care. Recent multicenter trials and updated 2025 ESC/EACTS guidelines highlight minimally invasive procedures’ positive impact on clinical outcomes and patient quality of life. Despite these successes, knowledge gaps remain regarding operator expertise, real-world adoption across institutions, criteria for patient selection, and how best to coordinate complex, multidisciplinary heart teams for the benefit of patients. This Research Topic aims to provide a dedicated forum to explore the current state and future perspectives of minimally invasive and hybrid interventions for adult valvular heart disease. By bringing together cardiac surgeons, interventional cardiologists, and general cardiologists, the Topic seeks to clarify best practices, refine patient selection, and discuss real-world results that inform clinical and procedural decision-making. The main objectives are to address whether these approaches should now be considered the standard of care or remain supplementary, to assess their safety and outcomes in diverse populations, and to analyze how collaborative models and innovative technologies can lead to more individualized, less invasive treatment strategies. This Topic focuses exclusively on adult valvular heart disease and valve-related structural interventions, deliberately excluding congenital heart disease, pediatric populations, and purely diagnostic innovations. It specifically encompasses contemporary procedural advances, clinical frameworks, and multidisciplinary strategies that enhance patient care in adult populations. To gather further insights into this evolving landscape, we welcome articles addressing, but not limited to, the following themes:o Comprehensive overview of minimally invasive surgical approaches in adult valve therapyo Advances in totally endoscopic cardiac surgery, including robotic and non-robotic techniqueso Criteria for patient selection and team-based decision-making in hybrid and transcatheter interventionso Clinical outcomes, safety data, and patient quality of life post-procedureo The role of ESC/EACTS guidelines and evolving evidence in refining clinical pathwayso Barriers and facilitators to the widespread adoption of minimally invasive and hybrid adult valvular interventionso Future perspectives: training, technology, and innovation in adult structural heart interventionsAppendix:We accept original research, systematic reviews, expert perspectives, clinical case reports, and technical reports relevant to the scope described.
