Innovations in Monitoring and Managing Pediatric Respiratory Conditions: From Infantile Wheeze to Neuromuscular Diseases

Respiratory disorders remain among the leading causes of morbidity in the pediatric population, spanning a remarkably heterogeneous spectrum, from common early-life conditions such as infantile wheeze and bronchiolitis, to chronic diseases like asthma and cystic fibrosis, to respiratory compromise secondary to neuromuscular diseases (e.g., spinal muscular atrophy, Duchenne muscular dystrophy). Despite differing etiologies, these conditions share a common clinical challenge: the need for accurate, timely, and minimally invasive monitoring to guide diagnosis, track disease progression, and tailor management strategies for a population whose airways, lung mechanics, and communication abilities differ fundamentally from adults.Recent years have seen rapid innovation in this space, wearable respiratory sensors, home-based pulmonary function testing, artificial intelligence-assisted interpretation of lung sounds and imaging, telemedicine-enabled remote monitoring, and advances in non-invasive ventilation for neuromuscular respiratory failure. These technologies hold the potential to transform care pathways, reduce hospitalization burden, and improve quality of life for children and their families. However, evidence remains fragmented across disease subtypes and age groups, and translation from research settings into routine pediatric practice is still uneven.GoalThis Research Topic aims to bring together clinical, translational, and technological perspectives on the monitoring and management of pediatric respiratory conditions across the full severity and etiological spectrum, from transient infantile wheeze to progressive neuromuscular respiratory disease. The goal is to consolidate current evidence, highlight emerging diagnostic and monitoring technologies, and identify gaps and opportunities that can inform both clinical practice and future research directions in pediatric respiratory care.Scope and Information for AuthorsWe welcome Original Research, Reviews, Mini Reviews, Case Reports, Clinical Trial articles, and Perspective articles addressing (but not limited to) the following themes:o Diagnostic and monitoring innovations for infantile wheeze and recurrent wheezing disorderso Novel biomarkers and lung function testing approaches in infants and young childreno Wearable and remote/telehealth-based respiratory monitoring technologieso Artificial intelligence and machine learning applications in pediatric respiratory diagnosticso Management strategies for chronic pediatric respiratory conditions (asthma, bronchopulmonary dysplasia, cystic fibrosis)o Respiratory monitoring and non-invasive/invasive ventilatory support in pediatric neuromuscular diseaseso Multidisciplinary and home-based care models for children with chronic respiratory needso Long-term outcomes and quality-of-life considerations in pediatric respiratory disease managemento Health equity and access challenges in pediatric respiratory monitoring technologiesWe particularly encourage submissions that bridge the gap between technological innovation and real-world clinical implementation, and that consider the full developmental trajectory from infancy through adolescence. Authors should ensure that clinical studies comply with relevant ethical approvals and reporting guidelines (e.g., CONSORT, STROBE, as applicable) and clearly state data availability.
