Innovations in Placenta Accreta Spectrum Care: From Etiology to Diagnosis and Management

Placenta Accreta Spectrum (PAS) disorders are among the most serious complications in contemporary obstetrics, associated with severe haemorrhage, surgical morbidity, preterm birth, and substantial impacts on maternal and perinatal outcomes. Rising caesarean delivery rates and other uterine interventions have contributed to an increasing burden of PAS worldwide, underscoring the need for improved prevention, earlier recognition, clearer pathological and clinical classification, and safer, evidence-based management pathways across diverse care settings. This Research Topic in the Obstetrics and Gynecology section of Frontiers in Medicine will bring together basic, clinical, translational, histopathological, and imaging-focused research that advances understanding of PAS etiology and pathophysiology, strengthens prenatal diagnosis and risk stratification, and improves management strategies from antenatal planning through intrapartum care and postpartum recovery. We particularly welcome multidisciplinary contributions that integrate obstetric imaging, histology, prediction modelling, surgical and interventional innovation, and maternal-foetal medicine approaches aimed at reducing morbidity and optimizing outcomes for pregnant patients and neonates. We also encourage contributions that examine the important and evolving debate around uterine scar dehiscence versus PAS, including diagnostic distinctions, overlapping features, terminology, and implications for clinical decision-making. In line with SDG 3: Good Health and Well-being, this collection seeks to support safer, more precise, and more equitable care for patients affected by abnormal placentation.Submissions may address, but are not limited to, the following themes:• Mechanisms and risk factors for PAS, including links to prior uterine surgery, placentation biology, and abnormal decidualisation• Histological and pathological correlates of PAS, including clinicopathological classification and concordance between imaging, surgical, and histological findings• Prenatal diagnosis and imaging innovations, including ultrasound and MRI protocols, reporting standards, and novel diagnostic modalities• Uterine scar dehiscence versus PAS: definitions, differential diagnosis, imaging features, intraoperative findings, histological interpretation, and implications for classification and management• Risk assessment tools, predictive models, and clinical pathways for referral, triage, and delivery planning• Surgical, conservative, and interventional strategies, with particular emphasis on uterine-conservative and uterus-preserving approaches, haemorrhage control techniques, focal resection, and adjunct technologies• Peripartum and postpartum outcomes, including transfusion strategies, critical care, fertility preservation, and recovery trajectories• Maternal, neonatal, and health-system outcomes across different populations, levels of expertise, and resource settings• Implementation science and multidisciplinary models of care, including PAS teams, simulation training, referral networks, and quality improvement initiativesWe welcome original research, systematic reviews, meta-analyses, clinical studies, methodological papers, and perspectives that help translate mechanistic, histopathological, and diagnostic advances into safer care and better short- and long-term outcomes for patients affected by PAS.
