Innovative Radiotherapy Strategies in Resource-limited Settings

Radiotherapy is a cornerstone of cancer management worldwide. However, significant disparities exist in delivering effective, high-quality radiotherapy to patients in resource-limited settings, including both low- and middle-income countries and underserved communities globally. Challenges such as gaps in infrastructure, workforce limitations, access to advanced technologies, and sustainable training inhibit optimal cancer care for many common malignancies, including cervical and breast cancer.Despite these hurdles, resource-limited environments are hubs of remarkable innovation, adapting techniques like brachytherapy, advanced imaging, and customized radiotherapy protocols to enhance outcomes within local constraints. Real-world solutions such as 3D-HDR brachytherapy for cervical cancer, novel strategies in breast cancer radiotherapy, and global partnerships that promote technology transfer and capacity building demonstrate transformative potential in the areas most in need. Our Research Topic intends to spotlight and speed the development, adaptation, and implementation of innovative radiotherapy strategies that are pragmatic, scalable, and impactful across all cancer types in resource-limited settings. We are keenly interested in real-world experiences, technical adaptations, cross-disciplinary collaborations, and health system innovations that advance progress in these constrained environments. We invite original research, reviews, methodology papers, case studies, and clinical perspectives addressing themes such as:• Adaptations and innovative radiotherapy techniques tailored for diverse cancers in resource-limited environments.• Advanced brachytherapy technique development and implementation with case studies or pilot initiatives in low- and middle-income regions.• Advanced imaging role and adaptation in treatment planning, delivery, and tumor delineation within technological or infrastructural constraints.• Challenges, lessons learned, and enabling factors in introducing novel protocols and technology in underserved regions.• International collaborations and partnership strategies involving education, technology transfer, and curriculum development, that enhance radiotherapy access and quality.• Healthcare systems research and policy for sustainability strategies, quality assurance, and outcomes improvement for cervical cancer, breast cancer, and other malignancies.• Equity in oncology addressing gender, global health, or culturally informed interventions for reducing disparities in cancer care and radiotherapy delivery. We strongly encourage submissions that include empirical evidence, innovative approaches, and transferable insights for advancing radiotherapy in all cancer forms within resource-constrained settings. Please note: manuscripts consisting solely of bioinformatics, computational analysis, or predictions of public databases which are not accompanied by validation (independent clinical or patient cohort, or biological validation in vitro or in vivo, which are not based on public databases) are not suitable for publication in this journal.Disclaimer: Topic Editor Onyinye Balogun is a stockholder and Former Co-Founder of Mission-Driven Tech.
