Integrating Artificial Intelligence, Multi-Omics, and Nanomedicine to Target the Disease Immune Microenvironment

The immune microenvironment plays a critical role in the progression and therapeutic resistance of a broad spectrum of diseases, most notably in cancers (such as hepatocellular, gastrointestinal, and urological carcinomas), inflammatory disorders, and auto immune diseases. While advanced immunotherapies and targeted agents offer immense clinical promise, their efficacy is often severely limited by complex biological barriers, poor tissue penetration, and adaptive immune evasion mechanisms.Overcoming these barriers requires a multidisciplinary convergence. Bioinformatics and machine learning are revolutionizing our ability to discover novel prognostic biomarkers and therapeutic targets from massive multi-omics datasets. Simultaneously, AI-assisted radiomics and deep learning models are enabling precise, non-invasive stratification of patients and real-time monitoring of treatment responses. To act upon these computational insights, advanced nanomedicine carrier systems—including lipid nanoparticles (LNPs), stimulus-responsive polymers, and extracellular vehicles —provide the necessary tools to encapsulate biomacromolecular therapeutics, immunomodulators, and small molecules, delivering them precisely to the targeted microenvironment. This Research Topic explores the intersection of big data, clinical oncology/immunology, and materials science. We aim to highlight research that uses computational and multi-omics approaches to guide the rational design of nanomedicines, as well as studies utilizing nanocarriers to modulate immune responses and overcome therapeutic resistance. We invite fundamental, preclinical, and translational studies that bridge these disciplines. The goal is to establish a modern translational pipeline: utilizing AI and bioinformatics to identify vulnerabilities in the disease microenvironment and deploying precision nanomedicine to effectively drug those targets. We encourage the following topics, but are not limited to:• AI & Omics-Guided Drug Delivery: Application of machine learning, single-cell sequencing, and multiomics to identify novel molecular targets for nanomedicine delivery.• Tumor Microenvironment Remodeling: Nanotherapy strategies designed to overcome immunosuppression, target myeloid-derived suppressor cells, or reverse resistance to immune checkpoint inhibitors (e.g., anti-PD-1 therapy).• Radiomics & Treatment Evaluation: Development of deep learning and multimodal radiologic-pathologic models to predict and evaluate patient responses to nanomedicines and neoadjuvant therapies.• Precision RNA Therapeutics: Design and pharmacological evaluation of siRNA, mRNA, and gene-editing carrier systems (e.g., LNPs) for localized treatment of cancers, autoimmune and inflammatory diseases.• Bio-responsive Nanocarriers: Development of smart nanomedicines (e.g., pH/ROS-responsive,zwitterionic) that specifically interact with and navigate disease-specific biological barriers.• Translational Biomarkers: Identification and validation of immune-related diagnostic markers that can serve as direct targets for next-generation nano-formulations.
