Interplay Between Menopause and Diabetes: Mechanisms, Risk Stratification, and Clinical Management

The menopausal transition represents a critical period of metabolic and endocrine change that can substantially influence the development, progression, and complications of diabetes. Declining sex hormone levels, changes in body composition, insulin resistance, inflammation, and alterations in cardiovascular and renal function collectively contribute to an increased risk of dysglycemia and type 2 diabetes in women during midlife and beyond. At the same time, diabetes may exacerbate menopause-related symptoms, cardiometabolic dysfunction, and long-term health outcomes, highlighting a complex and bidirectional relationship between these conditions. Despite growing recognition of this interplay, important questions remain regarding the underlying mechanisms, identification of high-risk individuals, and optimal approaches to prevention and management. This Research Topic aims to bring together multidisciplinary research that advances understanding of the interactions between menopause and diabetes across the continuum of disease. We welcome contributions exploring the endocrine, metabolic, cardiovascular, renal, and behavioral pathways linking menopausal transition with diabetes risk and progression, as well as studies investigating biomarkers, risk prediction tools, and personalized approaches to prevention and treatment. Particular emphasis will be placed on research that improves risk stratification, informs clinical decision-making, and enhances outcomes for women with prediabetes, type 2 diabetes, and related cardiometabolic disorders.Potential themes include, but are not limited to:• Endocrine and metabolic mechanisms linking menopause, insulin resistance, and dysglycemia• Risk factors and biomarkers for diabetes development across the menopausal transition• Prediabetes, type 2 diabetes, and cardiometabolic risk in peri- and postmenopausal women• Obesity, body composition changes, and metabolic syndrome during menopause• Roles of sex hormones, hormone therapy, and reproductive aging in diabetes risk and outcomes• Cardiovascular, renal, hepatic, and musculoskeletal complications of diabetes in menopausal women• Interactions between menopause, inflammation, and metabolic dysfunction• Precision medicine and personalized risk stratification approaches for women at risk of diabetes• Lifestyle, behavioral, and digital-health interventions for diabetes prevention and management during and after menopause• Clinical management strategies and guidelines for optimizing metabolic health in menopausal womenThis collection seeks to stimulate new research and clinical perspectives that improve understanding of diabetes in the context of reproductive aging and support evidence-based approaches to prevention, risk assessment, and patient care.
