Is Endocrine Dysregulation a Driver of Pulmonary Vascular and Parenchymal Remodeling?
Pulmonary vascular and parenchymal remodeling are defining histopathological features of pulmonary hypertension (PH) and chronic lung diseases, such as pulmonary fibrosis and chronic obstructive pulmonary disease (COPD). Yet the endocrine mechanisms that contribute to these structural and functional alterations remain incompletely integrated into mainstream cardiopulmonary research. Increasing evidence suggests that disturbances in systemic and local hormonal signaling influence pulmonary vascular tone, endothelial function, inflammation, fibrosis, metabolic reprogramming, and right ventricular adaptation. However, the field remains fragmented across subspecialties, limiting mechanistic synthesis and translational progress. This Research Topic aims to consolidate current knowledge on how endocrine dysregulation shapes pulmonary vascular and parenchymal remodeling, and right heart hypertrophy. Original and comprehensive review contributions integrating molecular, cellular, translational, and clinical perspectives are particularly encouraged, with clear links to lung vascular, parenchymal, or right ventricular endpoints. We welcome submissions addressing, but not limited to, the following themes:• Thyroid hormone signaling in pulmonary vascular tone and remodeling, and right ventricular function;• Sex hormones, menopause, and sex differences in pulmonary hypertension, lung fibrosis and COPD;• Renin–angiotensin–aldosterone system activation in pulmonary vascular and parenchymal remodeling;• Glucocorticoid and mineralocorticoid signaling in lung inflammation and pulmonary vascular remodeling;• Insulin resistance, adipokines, and metabolic dysfunction in pulmonary vascular and parenchymal diseases;• Neuroendocrine stress pathways and autonomic dysregulation in right heart adaptation;• Mitochondrial dysfunction and redox signaling as endocrine-modulated processes in the context of lung vascular and parenchymal disorders;• Immune–endocrine crosstalk driving pulmonary vascular and parenchymal remodeling;• Biomarkers of endocrine imbalance predicting the occurrence and progression of pulmonary vascular diseases;• Endocrine-targeted therapeutic strategies in pulmonary vascular and parenchymal disorders.By bringing together expertise in endocrinology, pulmonary vascular and parenchymal biology and medicine, cardiology, and translational medicine, this Research Topic seeks to advance a more integrated understanding of endocrine–cardiopulmonary interactions and to identify mechanistically informed strategies capable of improving outcomes in the context of severe, incurable and life-threatening pulmonary vascular and parenchymal disorders.
