Lipids and Therapeutic Targets for Cardio-Metabolic Diseases

Cardio-metabolic diseases, including obesity, type 2 diabetes, MASLD, and atherosclerotic cardiovascular disease, continue to account for a substantial global health burden across populations. At the core of these interrelated conditions, lipids act as dynamic signaling entities as well as energy reservoirs, influencing inflammation, endothelial function, insulin sensitivity, and tissue remodeling. Recent advances in lipidomics and spatial profiling have mapped an expanding array of bioactive lipid species and pathways, such as sphingolipids, ceramides, eicosanoids, and bile acids, that shape disease processes more accurately than traditional lipid measures. Ongoing work has elucidated the significance of these lipids in pathogenesis and highlighted potential targets for intervention, yet translating these discoveries swiftly and effectively into clinical care remains a key challenge.A persistent issue in cardio-metabolic medicine is the high residual disease risk that remains even after lipid-lowering therapies, underlining the need for precision biomarkers and targeted therapeutics. New evidence points to the heterogeneity in lipid-driven disease mechanisms and patient responses, making it essential to connect fundamental biological discoveries to strategies that can be deployed in everyday clinical practice. Bridging mechanistic research with clinical implementation could profoundly improve risk prediction, early detection, intervention, and patient outcomes, especially when harnessing innovations in analytic techniques, molecular profiling, and therapeutic development. This Research Topic aims to catalyze impactful advances in cardio-metabolic disease by uniting basic, translational, and clinical research focused on lipid biology, including lipoprotein metabolism, lipid signaling and lipid-induced metabolic consequences in cardiac cells. We invite contributions that not only reveal mechanistic pathways but also propose or evaluate actionable biomarkers and therapeutic strategies, with a focus on real-world applicability and precision medicine. A broad array of methodological approaches, including computational analysis, laboratory experimentation, clinical studies, and integrative translational projects, are encouraged to foster comprehensive understanding and clinical innovation. To gather further insights into the impact of lipid biology on cardiovascular and metabolic disease processes and outcomes, we welcome articles addressing, but not limited to, the following themes:• Lipidomics and lipoproteomics for cardio-metabolic risk stratification and patient subtyping;• Mechanistic roles of bioactive lipid pathways (e.g., ceramides, sphingolipids, oxylipins) in inflammation, insulin resistance, and atherosclerosis;• Lipoprotein biology beyond LDL-C (e.g., apoB, remnant cholesterol, Lp(a), HDL function) and novel roles in cardio-metabolic disease including heart failure;• Discovery, evaluation, and translation of innovative lipid-modifying therapies, including combination regimens and real-world effectiveness studies;• Molecular mechanisms of lipid signaling in cardiac remodeling, cardiac metabolism, heart failure subtypes (HFpEF/HFrEF), arrhythmias, and other cardiovascular conditions;• Microbiome–bile acid–lipid interactions as modulators and targets in cardio-metabolic disease epidemiology and patient outcomes.
