Multilevel Determinants of Cardiometabolic Risk: From Molecules to Behavior and Environment

Cardiometabolic diseases - including type 2 diabetes, obesity, hypertension, and dyslipidaemia - remain major contributors to global morbidity and mortality. Extensive research has explored individual determinants such as genetic predisposition, metabolic dysregulation, lifestyle behaviours, and environmental exposures. However, these factors rarely act in isolation. Emerging evidence highlights the importance of understanding cardiometabolic risk through a multilevel lens that integrates molecular mechanisms, behavioural patterns, and environmental contexts. Despite growing interest, significant gaps remain in linking mechanistic insights with population-level determinants, and in understanding how biological vulnerability interacts with lifestyle and environmental stressors across the life course. A comprehensive, systems-oriented approach is needed to better capture the complexity of cardiometabolic regulation and to inform more effective prevention and intervention strategies. This Research Topic aims to advance a holistic understanding of cardiometabolic health by integrating biological, behavioural, and environmental determinants within a multilevel framework. We seek contributions that explore how molecular pathways, endocrine regulation, and metabolic signalling interact with lifestyle behaviours, psychosocial stressors, and environmental exposures to shape cardiometabolic risk. By bridging mechanistic research with epidemiological and population-health perspectives, this collection aims to foster interdisciplinary dialogue and highlight context-specific vulnerabilities across diverse populations. We particularly welcome studies that address health inequalities, aging-related metabolic changes, and the interplay between genetic predisposition and modifiable risk factors. Ultimately, this Topic seeks to promote systems-level insights that can inform precision prevention, targeted interventions, and more equitable approaches to cardiometabolic health. This Research Topic welcomes original research, reviews, mini-reviews, systematic reviews, data-driven perspectives, and methodological papers. Submissions may address any of the following themes:• Molecular, endocrine and metabolic mechanisms underlying cardiometabolic regulation• Genetic, epigenetic, and inflammatory pathways• Behavioural determinants, including physical activity, nutrition, sleep, stress, and mental health• Environmental and social determinants, such as pollution, built environment, and socioeconomic conditions• Interactions across biological, behavioural, social and environmental levels• Systems epidemiology, multilevel modelling, and life-course approaches• Digital health technologies, big data, and novel methodological frameworksWe encourage contributions from diverse disciplines, including endocrinology, epidemiology, physiology, behavioural science, environmental health, and public health.
