Multimodal Imaging-Guided Theranostics in Neuroradiology: From Laboratory Design to Clinical Evidence, Infrastructure, and Utility
Theranostic platforms combine targeted therapy and diagnostic imaging into a single system. In neuroradiology, these platforms offer a powerful way to locate, treat, and monitor complex neurological diseases in real time. By integrating multimodal imaging methods such as positron emission tomography (PET), magnetic resonance imaging (MRI), ultrasound, and fluorescence, clinicians can track drug delivery and measure therapeutic response with high spatial precision.Despite these technological advances, translating theranostic platforms from laboratory models to clinical radiology departments remains a challenge. Implementation is often limited by a lack of specialized clinical infrastructure, high equipment and material costs, and a general shortage of Level I and Level II clinical evidence validating their usefulness and cost-efficiency. To bridge this gap, radiologists and scientists must work together to establish standardized clinical workflows, evaluate economic feasibility, and produce high-quality evidence. This Research Topic brings together research articles and reviews exploring the clinical translation of multimodal nano- and micro-theranostic systems in neuroradiology. We aim to highlight strategies that improve patient safety, demonstrate therapeutic benefit, and address the practical realities of clinical adoption. We particularly welcome studies that show how image-guided data analytics, pharmacokinetic modeling, and prospective clinical trials can accelerate the safe, cost-effective translation of these platforms. We invite contributions addressing, but not limited to, the following themes:- Clinical Evidence & Trial Design: Prospective clinical trials (specifically Level I and II evidence) evaluating the clinical utility, efficacy, and safety of theranostic agents in neuroradiology.- Infrastructure & Workflows: Operational requirements, safety protocols, and departmental infrastructure needed to deploy theranostic platforms in clinical radiology.- Economic Appraisals: Cost-benefit and cost-effectiveness analyses comparing theranostic workflows to traditional neuroimaging and therapeutic options.- Multimodal Integration: Integrating PET, MRI, ultrasound, and fluorescence for clinical treatment planning and therapeutic monitoring.- AI-Assisted Optimization: Clinical application of machine learning tools to optimize imaging-guided therapeutic agents and predict treatment responses.- Disease-Specific Applications: Translational applications of theranostic systems in neurology, neuro-oncology, and neurovascular medicine. We welcome high-quality submissions of all article types within scope, including FAIR² Data submissions, which enable datasets to be published as peer-reviewed, citable resources designed for reuse and AI-ready analysis.
