Multisystem Consequences of Psychological and Physical Trauma: From PTSD to Neurobiological Mechanisms and Neurorehabilitation

Psychological and physical trauma exert profound and long-lasting effects on the central and peripheral nervous systems. Post-traumatic stress disorder (PTSD), traumatic brain injury (TBI), polytrauma, and chronic pain frequently coexist with stress-related neuroinflammatory processes, producing complex neuropsychiatric and functional outcomes that challenge conventional diagnostic and therapeutic frameworks. While individual conditions such as PTSD and TBI have received considerable research attention in isolation, their overlapping pathophysiology, including shared neuroinflammatory cascades, neuroendocrine dysregulation, disrupted stress circuitry, and impaired neuroplasticity, remains insufficiently understood. The result is a fragmented evidence base that limits the development of integrative, mechanism-driven approaches to treatment and rehabilitation. This Research Topic aims to bring together basic, translational, and clinical research to advance a systems-level understanding of how psychological and physical trauma interact to produce neurological and neuropsychiatric dysfunction. By bridging mechanistic neuroscience with rehabilitation science, the collection seeks to clarify the integrative mechanisms linking stress exposure, structural brain injury, systemic dysregulation, and recovery trajectories. A central objective is to identify factors that drive resilience, maladaptation, and functional recovery, and to translate these insights into precision approaches for diagnosis, prognosis, and intervention. Contributions that connect bench findings to clinical or rehabilitative outcomes are especially valued. We welcome original research, reviews, systematic reviews, mini reviews, methods articles, and hypothesis and theory papers addressing the following themes:- Neuroinflammation and brain–immune interactions in trauma- Neuroplasticity and stress neurocircuitry- Neuroendocrine alterations following psychological and physical trauma- Blood-based and neuroimaging biomarkers for TBI, PTSD, and related conditions- Computational modeling of trauma-related neural processes- Neuromodulation and neurostimulation-based interventions- Robotic and digital rehabilitation technologies- Sex differences and aging in trauma recovery- Comorbid metabolic or cardiovascular dysregulationBy integrating perspectives across these domains, this Research Topic seeks to bridge the gap between pathophysiology and recovery, advancing precision approaches to trauma-related neurological and neuropsychiatric disorders.
