Myasthenia Gravis: Immunopathogenesis, Precision Diagnosis, and Targeted Management

Myasthenia gravis (MG) sits at the forefront of neuroimmunology as a prototypical antibody-mediated neuromuscular disorder. Advances in immune system profiling and serological testing have equipped clinicians to recognize and classify MG with greater accuracy, while targeted therapies, including monoclonal antibodies and pathway-specific agents, are expanding options beyond conventional immunosuppression and reshaping management guidelines. The clinical landscape is now characterized by more nuanced diagnostic strategies, a growing array of disease-specific therapeutic options, and a heightened need for practical frameworks that integrate evolving scientific knowledge into routine patient management. This fast-advancing field demands a dynamic and ongoing discussion that follows the pace of discovery and innovation, fostering new strategies for effective patient stratification, optimal sequencing of therapies, reliable biomarkers of response, and the clinical management of rare or overlapping syndromes, all with direct implications for clinicians delivering patient care. Major gaps remain: real-world effectiveness and safety data are limited, the prognosis of distinct MG variants is incompletely defined, and reliable clinical or laboratory markers to guide treatment choice are still needed. This Research Topic aims to consolidate current immunopathogenic and clinical insights and foster new evidence on the diagnosis, stratification, and management of myasthenia gravis, placing a strong emphasis on real-world challenges and frontline decision-making. Major objectives include evaluating the integration of immunological testing in diagnosis, stratifying patients for optimal therapeutic choices, and assessing outcome predictors within actual clinical practice. Furthermore, the topic will encourage the practical assessment of novel targeted therapies, management algorithms for complex or refractory patients, and strategies for recognizing and treating MG in the context of overlapping autoimmune or iatrogenic conditions. To gather further relevant insights in the immunopathogenesis, clinical care, and therapeutic management of myasthenia gravis, we welcome submissions across a diverse range of manuscript types supported by the journal—including Original Research, Brief Research Reports, Perspectives, Protocols, Mini-Reviews, and more—exploring, but not limited to, the following themes:- Advanced antibody and immunological testing: implementation, interpretation, and diagnostic limitations. - Stratification and management of MG subtypes, including seronegative and antibody-specific forms.- Immunopathogenic mechanisms with direct diagnostic or therapeutic relevance.- Biomarkers and predictors of disease course, treatment response, and safety.- Real-world use, sequencing, and long-term follow-up of targeted and biologic therapies.- Management of refractory MG and myasthenic crisis.- MG in special settings, including thymoma/paraneoplastic disease and immune checkpoint inhibitor exposure.- Diagnostic challenges and differential diagnosis of MG mimics in diverse care environments.- Patient-centered and multidisciplinary care, including validated outcome measures and patient-reported outcomes.Articles will be published online as soon as they are accepted after peer review and approved for publication.Sign up now to participate in this exciting Research Topic.Topic Editor Jana Zschüntzsch has received financial support for research from Amicus Therapeutics and Argenx.
