New Insights into the Pathophysiology and Treatment of Diabetes Complications

Diabetes mellitus (DM) is a highly prevalent metabolic disorder and a major global public health challenge. According to the International Diabetes Federation, approximately 589 million adults worldwide are currently living with diabetes, and this number is projected to reach 853 million by 2050. The major clinical subtypes are type 1 and type 2 diabetes mellitus (T1DM and T2DM). Diabetes can lead to severe complications, including neurological, cardiovascular, and kidney diseases, which substantially contribute to morbidity and mortality. The pathophysiology of diabetes complications is complex and involves systemic and tissue-specific metabolic, inflammatory, and hormonal alterations that promote cellular dysfunction, fibrosis, and progressive organ damage. Nevertheless, the underlying molecular and cellular mechanisms remain incompletely understood.Current therapeutic approaches include glucose-lowering agents, renin–angiotensin system inhibitors, sodium–glucose cotransporter 2 inhibitors, and other emerging therapies. However, their protective mechanisms and long-term effects remain incompletely defined, highlighting the need for novel therapeutic targets and pharmacological strategies. This Research Topic aims to deepen our understanding of the complex pathophysiological mechanisms underlying DM complications associated with both T1DM and T2DM. Among them, it includes diabetic neuropathy, diabetic cardiomyopathy, diabetic vascular disease, and diabetic kidney disease, from subclinical to established stages of the disease. In addition, this Research Topic also seeks to elucidate gaps in our understanding of the cellular and molecular mechanisms underlying the protective effects of established pharmacotherapies for DM complications. Furthermore, we aim to present novel findings and discuss promising therapeutic strategies, including emerging pharmacotherapies and their pharmacokinetic, pharmacodynamic, and safety profiles under both physiological and pathological conditions. In this context, we welcome submissions of original research, reviews, mini reviews, clinical trials, hypotheses and theories, and perspectives addressing, but not limited to, the following topics:-Preclinical studies using animal models and clinical studies involving patients with T1DM or T2DM;-Novel pathophysiological mechanisms underlying different DM complications, investigated in animal models and/or human studies;-Chronobiological mechanisms involved in DM complications and the impact of circadian rhythm disruption on their development and progression;-Pharmacokinetic and pharmacodynamic studies of novel therapeutic agents for the treatment of DM complications;-Safety and tolerability studies of emerging therapeutic compounds for DM complications;-Mechanistic studies investigating the effects of glucose-lowering agents, renin–angiotensin system inhibitors, SGLT2 inhibitors, antiproteinuric agents, and other established or emerging therapies;-Innovative pharmacological and non-pharmacological therapeutic strategies for the prevention or treatment of DM complications.
