New Technologies in Aortic Surgery

Aortic surgery encompasses rapidly evolving open, transcatheter, and endovascular approaches for the treatment of valvular and vascular disease. Technological advances in aortic valve prostheses, valve-repair devices, vascular grafts, endovascular stent-grafts, imaging, and procedural planning are expanding treatment options and enabling increasingly individualized care. However, important questions remain regarding the comparative effectiveness, durability, safety, and cost-effectiveness of emerging technologies, particularly in patients with complex anatomy, advanced age, or substantial operative risk. Recent studies have reported encouraging procedural and short-term outcomes for minimally invasive, transcatheter, branched, fenestrated, and hybrid interventions, while continued innovation is also improving open surgical reconstruction. Nevertheless, evidence remains heterogeneous, follow-up is often limited, and uncertainty persists regarding long-term outcomes, device durability, neurological and other procedure-related complications, and the selection of the most appropriate treatment for individual patients. This Research Topic will address these gaps by bringing together clinical, translational, engineering, and imaging research on contemporary and emerging technologies in aortic surgery. This Research Topic aims to evaluate how novel technologies are improving the treatment of aortic valve and vascular disease. It will highlight advances in device design, operative and endovascular techniques, imaging, clinical outcomes, and patient-specific treatment strategies. Contributions may also address the translation of preclinical innovations into clinical practice and compare emerging approaches with established open, endovascular, and hybrid procedures. To gather further insights into emerging technologies in aortic surgery, we welcome articles addressing, but not limited to, the following themes:• Novel aortic valve prostheses, including design, materials, haemodynamic performance, and durability• Devices and techniques for aortic valve repair and valve-sparing surgery• Innovative vascular grafts, conduits, prostheses, and hybrid surgical devices• New endovascular techniques and devices, including TEVAR and branched or fenestrated endografts• Next-generation delivery systems and minimally invasive procedural platforms• Comparative outcomes of open, endovascular, transcatheter, and hybrid approaches• Advanced imaging, three-dimensional modelling, and technologies for procedural planning• Patient-specific device selection and treatment strategies• Perioperative, neurological, renal, and other procedure-related outcomes• Long-term follow-up, device durability, reintervention, and real-world registry data• Preclinical research and early clinical evaluation of emerging technologies• Health economics, implementation, training, and regulatory considerationsThe Research Topic welcomes original research, reviews, case reports, and perspective articles.** Topic Editor Prof. Francesco Onorati has received a research grant from Edwards Lifesciences and an invited speaker.
