Next-generation Immunomodulatory Strategies in Periodontal and Peri-implant Disease
Periodontitis and peri-implantitis are among the most common inflammatory conditions affecting oral health. Despite advances in mechanical, surgical, and regenerative treatment, clinical outcomes remain difficult to predict, and some patients continue to experience disease progression, recurrent inflammation, or treatment failure. Increasing evidence indicates that tissue destruction is driven by complex interactions between the microbial challenge, the host immune response, genetic and environmental factors, and systemic health. This has stimulated interest in immunomodulatory strategies that target dysregulated inflammation and tissue responses, with the aim of achieving more predictable, individualized, and safer disease control. This Research Topic welcomes original research, reviews, systematic reviews, meta-analyses, and perspective articles addressing the development, evaluation, and clinical translation of next-generation immunomodulatory approaches in periodontal and peri-implant disease. Topics of interest include, but are not limited to:· Clinical, microbial, immunological, molecular, and genetic markers for predicting treatment response and disease progression in periodontitis· Biomarkers for identifying patients at risk of recurrent disease, periodontal breakdown, or peri-implantitis· Mechanisms underlying persistent inflammation and treatment resistance in periodontal and peri-implant disease· Host-microbiome interactions and their contribution to disease initiation, progression, and therapeutic response· Challenges in the treatment of peri-implantitis, including the limitations of current non-surgical, surgical, and regenerative approaches· Strategies for preserving osseointegration and promoting soft- and hard-tissue healing around dental implants· Host-modulation therapies targeting immune, inflammatory, bone-resorptive, and tissue-repair pathways· Systemic and locally delivered pharmacological approaches for controlling periodontal and peri-implant inflammation· Novel drug-delivery systems, biologics, nanoparticles, and other therapeutic platforms· Strategies for achieving predictable medical control of periodontal inflammation with minimal adverse effects· Safety, tolerability, and long-term outcomes associated with emerging immunomodulatory treatments· Combination therapies integrating immunomodulation with mechanical debridement, surgery, antimicrobials, or regenerative procedures· Personalized and precision approaches to periodontal and peri-implant therapy· Clinical trial design, translational models, and outcome measures for evaluating immunomodulatory interventionsStudies that connect mechanistic findings with clinical decision-making are particularly encouraged, as are contributions addressing the management of patients who respond incompletely to conventional periodontal or peri-implant therapy.
